Objectives To investigate the relationship between the level of free triiodothyronine(FT3)and coronary heart disease and its severity.Methods The clinical data of 171 patients who underwent coronary angiography in the Department 1 of Cardiovasology,North China University of Science and Technology Affiliated Hospital from February 2020 to February 2022 were collected,and all patients were divided into control group(n=74)and coronary heart disease(CHD)group(n=97),and all patients in CHD group underwent virtual histological intravascular ultrasound(VH-IVUS).According to clinical manifestations,the CHD group was divided into stable angina pectoris(SAP)group(n=44)and acute coronary syndrome(ACS)group(n=53).According to the median level of FT3,CHD patients were divided into high FT3 group(n=50)and low FT3 group(n=47).The basic data and laboratory examination indexes of all patients were collected,the coronary angiography results of all patients were analyzed,and the Gensini score of patients in the CHD group was calculated.Analysis of VHIVUS parameters of patients in CHD group: maximum plaque thickness,minimum plaque thickness,eccentricity index,external elastic membrane area(EEM),mean EEM of reference segment,remodeling index,minimum lumen area(MLA),plaque burden(%),necrotic core(NC)area percentage,dense calcium(DC)area percentage,fibrous(FI)area percentage,fibro-fatty(FF)area percentage,measured within the volume segment of interest,NC volume percentage,DC volume percentage,FI volume percentage,FF volume percentage and plaque volume were evaluated to evaluate whether the plaque was inflammatory thin fibrous cap atherosclerotic plaque(TCFA).Results 1.The number of smoking history,hypertension and diabetes in the CHD group were higher than those in the control group,and the difference was statistically significant(P<0.05).2.The low density lipoprotein of CHD group was significantly higher than that of control group,and the level of FT3 was significantly lower than that of control group,and the difference was statistically significant(P<0.05).3.The level of FT3 in the ACS group was lower than that in the control group,and the difference was statistically significant(P<0.05).4.With CHD as dependent variable and various clinical data as independent variables,multivariate Logistic regression analysis found that diabetes,hypertension,high LDL level and low FT3 level were independent risk factors for CHD.5.The Gensini score of the low FT3 group was lower than that of the high FT3 group,and the difference was statistically significant(P<0.05).6.The remodeling index of low FT3 group was higher than that of high FT3 group,and the MLA was lower than that of high FT3 group,and the difference was statistically significant(P<0.05).7.The volume percentage of FI in the high FT3 group was higher than that in the low FT3 group,and the difference was statistically significant(P<0.05).8.The number of TCFA plaques in the low FT3 group was higher than that in the high FT3 group,and the difference was statistically significant(P<0.05).9.The ROC curve showed that the AUC of FT3 for predicting TCFA plaques was 0.671(95%CI: 0.560-0.782,P<0.01),and when the critical value was 4.485mmol/L,the diagnostic sensitivity was 59.60%,and the specificity was80.00%,and the Youden index was 0.396.Conclusions 1.Low FT3 level can be used as an independent risk factor for CHD.2.CHD patients with low FT3 level have higher Gensini score,larger remodeling index,smaller minimum lumen area and less fiber content in plaque.3.FT3 level is helpful to predict TCFA.Figure 2;Table 11;Reference 147...
