Clinical Study Of Thunderbolt Moxibustion Combined With Vacuum Sealing Drainage Technique In The Treatment Of Diabetic Foot Wounds

Objective:To observe the clinical effect of thunderbolt moxibustion combined with vacuum sealing drainage technique in the treatment of diabetic foot wounds,and to explore the feasibility and effectiveness of this method acting on clinical practice,and then enrich the clinical treatment of diabetic foot wounds to provide a new research direction.METHODS:According to the set inclusion and exclusion criteria,30 patients with diabetic foot ulcer wounds admitted to the Department of Trauma,Department of Orthopaedics and Hand Surgery,Ruikang Hospital Affiliated to Guangxi University of Traditional Chinese Medicine from February 2022 to March 2023 were selected and divided into thunderbolt moxibustion combined with VSD group(n=15)and simple VSD group(n=15)according to different treatment regimens.Vacuum sealing drainage was performed for all patients after perfecting the relevant examinations to rule out surgical contraindications.The patients in the control group were postoperatively given anti-infection,swelling relief,anticoagulation and other symptomatic treatment,and active treatment of the underlying diseases.The patients in the observation group started to use thunderbolt moxibustion external treatment technique from the first day after surgery to 14 days after surgery,once a day,each time moxibustion Shenque,bilateral Zusanli three acupoints,the rest were the same as the control group.White blood cells,C-reactive protein(CRP),erythrocyte sedimentation rate(ESR)and IL-6(interleukin-6)were collected before operation,on the 7th day after operation and on the 14 th day after operation.The TCM symptom score,TCM symptom efficacy,ulcer area and ulcer healing rate were collected before and after treatment.The efficacy of diabetic foot ulcer wound was compared between the two groups by statistical analysis.Results:There was no significant difference in the baseline data between the two groups(P>0.05),with comparability.PCT,CRP,ESR and IL-6 in the two groups on the 7th and 14 th day after operation were lower than those before operation,and the differences had statistical significance(P<0.05);PCT,CRP and ESR in the two groups were significantly better than those in the control group on the 7th and 14 th day after operation,and the differences had statistical significance(P<0.05);IL-6 in the two groups on the 7th and 14 th day after operation was lower than that before operation,and the differences had statistical significance(P<0.05).EGF and b FGF in the two groups were lower than those before operation,and the differences were statistically significant(P<0.05);EGF and b FGF in the two groups were significantly different on the 14 th day after operation(P<0.05).The ulcer area at 7 and 14 days after operation in both groups was lower than that before operation,and the differences had statistical significance(P<0.05).The difference in the ulcer area at 7 and 14 days after operation between the two groups had no statistical significance(P>0.05).There was significant difference in ulcer healing rate after treatment between the two groups(P<0.05).There was no significant difference in preoperative TCM symptom scores between the two groups(P>0.05),and the TCM symptom scores in the observation group were lower than those in the control group at 14 days after surgery,and the difference was statistically significant(P<0.05).The total effective rate of the control group was 80%.The total effective rate of the observation group was93.3%.The difference was statistically significant(P<0.05).Conclusions:Thunderbolt moxibustion combined with negative pressure drainage is superior to VSD alone in promoting the healing of diabetic foot ulcer,improving the symptoms and signs,and regulating the level of inflammatory factors.No obvious adverse reactions occurred in the process of external treatment with thunderbolt moxibustion,indicating that this method is safe and reliable,and can be better combined with negative pressure drainage technology for treatment,and easy to operate,wide range of application,worthy of clinical promotion.
