ObjectiveTo understand the current situation of symptom burden,sense of coherence and quality of life in patients with chronic heart failure.To explore the correlation between symptom burden,sense of coherence and quality of life,study the role of sense of coherence between symptom burden and quality of life,enrich the correlation study of sense of coherence,and provide theoretical reference for improving chronic heart failure quality of life.MethodsUsing the cross-sectional study method,a total of 181 patients with chronic heart failure were selected from a Grade-A hospital in Dalian City from January 2022 to November 2022 by convenience sampling method.The survey tools included self-designed general data questionnaire,Memorial Symptom Assessment Scale(MSAS),Sense of Coherence Scale(SOC-13),and Minnesota Living with Heart Failure Questionnaire(MLHFQ).The data were input into Excel software,and descriptive statistics,one-way variance analysis,independent-samples T test,Pearson correlation analysis and multiple linear regression analysis were carried out by SPSS 26.0.Structural equation models were constructed by means of Amos25.0,and methods such as path analysis were used to explore whether there was an intermediary for statistical analysis and sorting of data.Results1.Symptom burden score of patients with chronic heart failure was(0.86±0.35),and the difference of symptom burden of patients with different self-care ability,ejection fraction and cardiac function grade was statistically significant(P < 0.05).Sense of coherence score was(63.73±14.68),sense of coherence of patients with different marital status,self-care ability and heart function classification had statistical significance(P < 0.05).Quality of life score was(43.95±19.95),and the total score of quality of life of patients with different working conditions,self-care ability,duration of disease and cardiac function grade had statistical significance(P < 0.05).2.Correlation analysis showed that symptom burden score of patients with chronic heart failure was negatively correlated with the total score of sense of coherence(r=-0.424,P < 0.05),Symptom burden score of patients with chronic heart failure was positively correlated with quality of life score(r=0.626,P < 0.01).The score of sense of coherence of patients with chronic heart failure was negatively correlated with the score of quality of life(r=-0.720,P < 0.01).3.Multiple linear regression analysis showed that self-care ability,course of disease,symptom burden,and sense of coherence were the main factors affecting the quality of life of patients with chronic heart failure,which together explained 68.5% of the variability.4.The results of mediating effect test showed that sense of coherence had a partial mediating effect between symptom burden and quality of life in patients with chronic heart failure,accounting for 37.74% of the total effect.AMOS was used to construct the structural equation model and the fitting indexes were CMIF/DF=1.946,GFI=0.967,CFI=0.989,AGFI=0.916,IFI=0.989,RMSEA=0.072.The Bootstrap test showed that the 95% confidence intervals of total effect,direct effect and indirect effect did not contain 0,indicating the existence of partial mediation.Conclusion1.The symptom burden of patients with chronic heart failure is heavier,which is mainly affected by self-care ability,ejection fraction and cardiac function grading.Sense of coherence was low,which was mainly affected by marital status,self-care ability and cardiac function classification.The quality of life is at a medium to low level,mainly affected by work conditions,self-care ability,disease course,cardiac function classification.2.Symptom burden,sense of coherence and quality of life in patients with chronic heart failure are related.Symptom burden can negatively predict quality of life,and sense of coherence can positively predict quality of life.3.Sense of coherence plays a partial mediating role between symptom burden and quality of life in patients with chronic heart failure.Improving sense of coherence can alleviate the negative impact of symptom burden on quality of life.Nursing workers should adopt systematic,individual and effective physical and mental care programs to help patients relieve the burden of symptoms and improve the level of psychological congruence,so as to improve the quality of life of patients.
