| Objective: To investigate the clinical efficacy of Huoxue Quba granules combined with erbium-doped fiber laser in the treatment of acne atrophic scar by randomized controlled trial.Methods: patients diagnosed as acne atrophic scar and blood stasis syndrome were selected from the outpatient department of cosmetic dermatology,affiliated Hospital of Shandong University of traditional Chinese Medicine from December 2021 to December 2022.A total of 63 patients who met the inclusion criteria and volunteered to participate in this study were randomly divided into two groups,including 32 patients in the treatment group and 31 patients in the control group.The treatment group was treated with Huoxue Quba granule combined with 1550 nm erbium-doped fiber laser,while the control group was treated with only 1550 nm erbiumdoped fiber laser.Take 4 weeks as a course of treatment,a total of 3courses of treatment were observed.Before the first treatment,4 weeks after treatment,8 weeks after treatment and 12 weeks after treatment,the facial lesions of the patients were photographed from the front,45°on the left and 45°on the right under the same light at the same position.The facial lesions of the patients were scored by a fixed cosmetic dermatologist who did not participate in this study according to the Acne weight score scale(ECCA).After 12 weeks of treatment,the efficacy index was calculated according to ECCA score.The adverse reactions during the treatment were recorded in detail,and the patients were asked to fill in the self-satisfaction questionnaire after 12 weeks of treatment.In the course of treatment and follow-up,3 cases fell off due to their own reasons,and finally completed a total of 60 cases(treatment group 30 cases,control group 30 cases).The data were analyzed by SPSS27.0statistical software.Results:(1)There were no significant differences in baseline data(gender,age,course of disease,ECCA score)before treatment between the two groups(P>0.05),indicating comparability.(2)Comparison within the ECCA score group: ECCA scores in 2 groups were significantly decreased after 12 weeks of treatment compared with before treatment(P<0.05),indicating that the two methods have achieved significant clinical efficacy in the treatment of facial acne atrophic scar.(3)Comparison of ECCA scores between the two groups: After 4 weeks of treatment,the ECCA score of the treatment group and the control group was compared(P>0.05),and the difference was not statistically significant.After 8 weeks of treatment and 12 weeks of treatment,the difference of ECCA scores between the treatment group and the control group was statistically significant(P<0.05).The difference of ECCA scores before and after treatment between the two groups was statistically significant(P<0.05).It showed that the clinical effect of Chinese medicine combined with laser therapy was better than that of single laser therapy.(4)The markedly effective rate of the treatment group(83.33%)was significantly higher than that of the control group(23.33%),and the total effective rate of the treatment group(96.67%)was significantly higher than that of the control group(80.00%),and the clinical effect of the treatment group was better than that of the control group(P<0.05).(5)The total satisfaction rate of the treatment group was93.33% higher than that of the control group 80.00%,and the difference in subjective satisfaction between the two groups was statistically significant(P<0.05).The results showed that the therapeutic effect of Chinese medicine combined with laser therapy group could better meet patients’ psychological expectations and was more acceptable to patients.(6)During the whole treatment process,there were no systemic adverse reactions in the two groups,and there were no infections and depigmentation in the treatment area.The results showed that the combined treatment group had high safety and and less side effects.Conclusion: Huoxue Quba Granule combined with erbium-doped fiber laser in the treatment of acne atrophic scar with blood stasis has significant clinical efficacy,and has advantages such as fewer side effects,high safety and good patient compliance.The main therapeutic principle of Huoxue quba granules is to promote blood circulation and remove blood stasis.The 1550 nm erbium-doped fiber laser can form column micro-thermal damage zones(MTZs)in the deep skin epidermis and dermis,and protect the integrity of the corneum layer.The combination of the two reflects the therapeutic idea of combining Chinese and Western medicine with internal and external treatment,and has achieved significant clinical therapeutic effect.Huoxue Quba Granule combined with erbium-doped fiber laser is an effective means to treat atrophic scar of acne with blood stasis,which is worthy of further study and promotion as a routine clinical treatment. |
