| Objective: To observe the clinical efficacy of nourishing yin and activating blood circulation prescription in the treatment of early diabetic nephropathy with yin deficiency and stasis heat type by nourishing liver and kidney,clearing heat and activating blood circulation.Content:This study is a clinical observational study.It mainly collects patients from November 2021 to November 2022 in the Department of Internal Medicine and Comprehensive Internal Medicine of Shandong Provincial Hospital of Traditional Chinese Medicine.After the collection is completed,statistical methods are used for analysis and statistics,and finally conclusions are drawn.According to the form of group control,the clinical effect of Ziyin Huoxue Decoction in the treatment of early diabetic nephropathy with Yin deficiency and blood stasis heat type was analyzed and compared.The results of the comparison between the basic treatment of the control group and the clinical efficacy of the experimental group with Ziyin Huoxue Decoction were obtained by analyzing and comparing the blood glucose index,blood lipid index,urine protein index,renal function index and TCM syndrome score before and after treatment.In addition,this study also summarizes the pathogenic factors and pathogenesis of diabetic nephropathy,the understanding of the disease name,etiology and pathogenesis,therapeutic principles and methods of traditional Chinese medicine,and the unique insights of the tutor on this disease.Methods: A total of 64 patients who met the inclusion criteria were divided into experimental group and control group by random parallel control method,with 32 patients in each group.After 8 weeks of treatment,a total of 5 cases were eliminated,shed,and suspended during the period.Finally,the experimental group and the control group each had 30 cases,and a total of 60 cases met the research requirements.On the basis of the general treatment of the control group and the treatment plan of conventional western medicine,the experimental group was treated with the traditional Chinese medicine prescription of nourishing yin and activating blood circulation.The fasting plasma glucose(FPG),2 hour postprandial blood glucose(2h PG),glycosylated hemoglobin(Hb A1c),low density lipoprotein-cholesterol(LDL-C),urinary albumin /creatinine ratic(UACR),urinary microalbuminuria(m ALB),cystatin C(Cys)and TCM syndrome scores were recorded before and after treatment in the experimental group and the control group.The corresponding statistical methods were used to analyze and discuss different data.Results: The results showed that the FPG,2h PG,Hb A1 c,LDL-C,UACR,m ALB,Cys and TCM syndrome scores of the experimental group and the control group were significantly improved after treatment(P < 0.01).After treatment,the comparison between the experimental group and the control group(P < 0.05)was statistically significant,indicating that the experimental group was better than the control group in improving the relevant indicators;compared with the control group,the curative effect of TCM syndromes in the experimental group was better(P < 0.05).Conclusion:Compared with simple western medicine treatment,Ziyin Huoxue Decoction combined with western medicine basic treatment is more effective in treating patients with early diabetic nephropathy of Yin deficiency and blood stasis heat type.The prescription of nourishing yin and activating blood circulation is obtained by the tutor combined with the thoughts of the ancients and his own clinical experience,with nourishing liver and kidney,clearing heat and activating blood circulation as the general principle,to be treated.It can not only improve the pain of patients ’ clinical symptoms,but also treat the disease from the pathogenesis.It has excellent curative effect and no obvious adverse reactions.Because of its good safety and innovative clinical application value,it has far-reaching reference significance for clinical practice and is worthy of further study and discussion. |
