| Objective:To evaluate the efficacy and safety of Yuyin Qianyang Decoction in the treatment of common adverse reactions of endocrine therapy for prostate cancer.Methods:64 patients with prostate cancer who had adverse reactions after routine endocrine therapy were included in the study.64 patients with prostate cancer were treated with Yuyin Qianyang Decoction while receiving routine endocrine therapy.The improvement of adverse reactions of endocrine therapy was observed after 4,8 and 12 weeks of treatment with Yuyin Qianyang Decoction.Hot flashes diary was used to evaluate the frequency of tidal fever,the severity of tidal fever and the change of Hot flashes score.The TCM symptom score table evaluates the improvement of symptoms such as hot flashes,sweating,irritability,and fatigue,and objectively evaluates the total effective rate after the improvement of TCM symptoms;the quality of life of patients with prostate cancer was evaluated by EORTC QOL C-30853.The safety and efficacy of Yuyin Qianyang Decoction in the treatment of prostate cancer patients were evaluated by detecting the changes of prostate specific antigen(PSA),testosterone(T),The changes of liver and kidney function,blood routine and other indicators were used to evaluate the safety and efficacy of Yuyin Qianyang Decoction in patients with prostate cancer.Results:Among the 64 patients with prostate cancer,12(18.75%)were 53-64 years old and 52(81.25%)were 65-85 years old.BMI:normal BMI accounted for 56.25%,obese accounted for 37.5%,obese accounted for 4.69%.Elevated PSA(53.13%)and dysuria(39.06%)were the chief complaints of most patients with prostate cancer.Pathological types:acinar adenocarcinoma(96.88%),ductal adenocarcinoma in 2 cases(3.125%);pathological stage:range from T2a to T4,of which T2c(23 cases,35.94%)accounted for the vast majority,bone metastasis(38 cases,59.38%),lymph node metastasis(17 cases,26.56%);gleason grade:6-10 points,4+4=8 points(16 cases,25%).The baseline duration of endocrine therapy was 13.8(5.5-24)months,with a median duration of 13.8 months.Endocrine therapy:mainly luteinizing hormone releasing hormone agonist+antiandrogen drugs(46 cases,71.88%).Diary of hot flashes:After 8 and 12 weeks of intervention with Yuyin Qianyang Decoction,the frequency of hot flashes was significantly lower than that before treatment(P<0.0001),and the difference was statistically significant;after 4,8 and 12 weeks of intervention with Yuyin Qianyang Decoction,mild and severe hot flashes were significantly reduced compared with those before treatment(P<0.05,P<0.01),and the difference was statistically significant.After 4,8 and 12 weeks of intervention with Yuyin Qianyang Decoction,the tidal fever score was significantly lower than that before treatment(P<0.0001),and the difference was statistically significant.TCM symptom score,efficacy evaluation:liver and kidney yin deficiency,liver yang hyperactivity syndrome,the total effective rate of TCM symptoms was 42.19%.Among them,hot flashes(60.94%),five upset hot(64.06%),irritability(64.06%)effective rate reached more than 60%;the effective rate of sweating(54.69%),dry mouth and throat(59.38%),fatigue(50%),lumbar and knee weakness(50%)was 50%or more.The effective rate of insomnia(43.75%),loss of appetite(42.36%),head pain(35.94%),head heavy foot(34.37%),chest pain(31.25%),other parts of the pain(31.25%)reached more than 30%.The effective rate of dizziness and tinnitus(23.44%)and defecation(29.69%)was more than 20%.Prostate cancer quality of life score:After 4,8 and 12 weeks of intervention,the overall health status of prostate cancer patients was significantly improved compared with that before treatment(P<0.0001),and the difference was statistically significant.After 4,8 and 12 weeks of intervention,the physical condition of prostate cancer patients was significantly improved compared with that before treatment(P<0.0001).After 8 and 12 weeks of traditional Chinese medicine treatment,the symptoms of urination,emotion and pain in patients with prostate cancer were significantly improved compared with those before treatment(P<0.0001),and the difference was statistically significant.After 8 and 12 weeks of Chinese medicine intervention,the physical function and social living conditions of the patients were reduced compared with those before treatment(P<0.01,P<0.0001),and the difference was statistically significant.Yuyin Qianyang Decoction has no obvious improvement on sexual function.Laboratory examination:After 4,8 and 12 weeks of intervention with Yuyin Qianyang Decoction,PSA was significantly lower than that before treatment,and T showed an upward trend compared with that before treatment(P>0.05).After 4,8 and 12 weeks of intervention,red blood cells,hemoglobin and white blood cells were lower than those before treatment(P>0.05),and the difference was not statistically significant.Platelets decreased after 4 and 8 weeks of treatment and increased after 12 weeks of follow-up(P>0.05).After 8 and 12 weeks of treatment,ALT and AST were higher than those before treatment,and decreased after 4 weeks(P>0.05).After 4,8 and 12 weeks of intervention,there was no significant change in creatinine and urea compared with before treatment(P>0.05),and the difference was not statistically significant.Adverse reactions:After 12 weeks of treatment,the incidence of moderate anemia in prostate cancer increased by 3.13%compared with that before treatment.It is considered that moderate anemia may have nothing to do with traditional Chinese medicine intervention.Conclusion:After the intervention of Yuyin Qianyang Decoction on the adverse reactions of endocrine therapy for prostate cancer,the frequency,degree and score of hot flashes in patients were improved to varying degrees,and the total effective rate of hot flashes was 60.94%.At the same time,it can improve the symptoms of five upset fever,irritability,sweating,dry mouth and dry throat,fatigue and other symptoms in patients with prostate cancer after endocrine therapy;after the intervention of traditional Chinese medicine,the quality of life of patients with prostate cancer was improved,mainly in emotional status and physical condition;after the intervention of traditional Chinese medicine,PSA in patients with prostate cancer showed a decreasing trend.This prescription will not promote the progress of the disease.Yuyin Qianyang Decoction is safe and effective in intervening in endocrine adverse reactions of prostate cancer. |
