| Objective This study through analysis of the application value of pulmonary ultrasound in neonatal respiratory distress syndrome(NRDS)diagnosis and treatment,so as to provides reference for the clinical treatment of NRDS.Methods A total of 73 children with NRDS admitted to the neonatal intensive care unit of QUZHOU People’s Hospital from Oct.2020 to Jun.2022 were analyzed and included in the observation group(n=73),and 50 healthy newborns during the same period were selected and included in the control group(n=50).The clinical data and pulmonary ultrasound characteristics of the two groups were compared,and the diagnostic value of pulmonary ultrasound and X-ray in children with NRDS was compared.Subsequently,the lung ultrasound score of of healthy newborns and children with NRDS were compared,as well as those of children with different disease degrees,and the correlation between the lung ultrasound score and X-ray grading of NRDS children was analyzed.Combined with the prognosis of children with NRDS,the value of lung ultrasound score in predicting the prognosis of children with NRDS was analyzed.Results There were no significant differences in gender,gestational age,age,body weight,length and other general information between healthy newborns and NRDS children(P>0.05).The pulmonary ultrasonography of healthy newborns showed that both lungs were line A with sporadic line B,the pleural line was regular and smooth,and the lung slip sign was normal.The pulmonary ultrasonography of NRDS newborns showed that line A of both lungs disappeared,line B was densely distributed or discontinuous,showing waterfall sign,and diffuse severe consolidation could be seen in both lungs,among which 32 newborns could see weakened pulmonary slip sign,27 newborns could see unsmooth pleural line and irregular thickening,23 newborns could see signs of lung consolidation,and 5 newborns had a small amount of pleural effusion.The AUC of pulmonary ultrasound and X-ray diagnosis of NRDS were 0.973 and 0.986,the sensitivity were 94.52%and 97.26%,and the specificity were 92.00%and 96.00%,and there was no statistical significance in the diagnostic value,sensitivity and specificity between them(P>0.05).The lung ultrasound score of whole lung,left lung,right lung and bilateral lung in observation group were significantly higher than those in control group(P<0.05).There were statistically significant differences in the lung ultrasound score of NRDS children with different disease degrees(F=11.483,P<0.05),that is,with the aggravation of the disease,the lung ultrasound score of NRDS children increased gradually.There were statistically significant differences in the lung ultrasound score of NRDS children with different X-ray grades(P<0.05).Spearman correlation analysis showed that the lung ultrasound score of NRDS children were positively correlated with X-ray grades(r=0.785,P<0.05).Among the 73 children with NRDS,17 had poor prognosis,and 56 had good prognosis.The AUC for predicting prognosis by lung ultrasound score and X-ray was 0.849 and 0.860,the sensitivity was 83.93%,96.43%,and the specificity was 76.47%and 70.95%.Conclusions(1)The pulmonary ultrasound findings of NRDS children were significantly different from those of healthy newborns.Compared with healthy newborn,the pulmonary ultrasound performance of NRDS children was significantly lower.(2)The diagnostic value of pulmonary ultrasound in NRDS is similar to that of X-ray,which can provide reference for the clinical diagnosis of NRDS.(3)The lung ultrasound score of children with NRDS was significantly different from that of healthy newborns,and with the aggravation of the severity of NRDS,the lung ultrasound score of children with NRDS increased gradually.The difference of the lung ultrasound score of children with NRDS with different X-ray grades was significant,and the lung ultrasound score of children with NRDS was positively correlated with the X-ray grade,suggesting that the lung ultrasound score could provide a reference for the evaluation of the severity of NRDS.(4)The lung ultrasound score has a high predictive value for the prognosis of children with NRDS,and can provide reference for the prognosis evaluation of children with NRDS. |
