The Efficacy And Safety Of Jianpigushenhuoxue Decoction Combined With Western Medicine In The Treatment Of Early DN

Objective:Diabetic nephropathy(DN)is one of the common complications of diabetes,with a high rate of death and disability,which seriously affects the quality of life of patients.Irirreversible renal impairment has not occurred in patients with early DN,and active treatment plays an important role in improving prognosis.This study discussed and analyzed the efficacy and safety of Jianpigushenhuoxue decoction combined with western medicine in the treatment of early DN,in order to provide scientific basis for improving the clinical diagnosis and treatment level of DN.Methods:In this study,108 early DN patients admitted to the Department of Nephrology from March 2020 to March 2022 in Wenzhou Municipal People’s Hospital were selected as the study subjects and divided into two groups:reference group and test group,with 54 cases in each group.The reference group used the conventional western medicine intervention,and the test group used the Jianpigushenhuoxue decoction combined with the conventional western medicine intervention.After 8 weeks of treatment,To evaluate the overall clinical efficacy of both groups,Record to compare the changes in TCM syndrome points between the two groups,To evaluate the efficacy of TCM syndrome,And measured and compared the glycemic indexes[glycated hemoglobin(HbA 1 c),fasting blood glucose(FBG),2h postprandial blood glucose(2 h PBG)],blood lipid index[triglycerides(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C)(LDL-C))and renal injury index[serum cystatin C(Cys-c),creatinine(Scr),24h urinary protein quantification(24-UPRO)],and the adverse reactions during the treatment were counted to analyze the safety of medication.Results:(1)After 8 weeks of treatment,the total effective rate of patients in the test group was 86.54%,and the total effective rate of the reference group was 65.38%.The clinical efficacy of the test group was higher than that of the reference group,and the difference was statistically significant(P<0.05).(3)There was no statistical difference between the two groups(P>0.05).After 8 weeks of medication,the score of the two groups decreased to different degrees compared with before medication(P<0.05),and the score after medication was lower than that of the reference group,and the difference was statistically significant(P<0.05).The total effective rate of TCM syndrome in the test group patients was 90.38%,and the total effective rate of TCM syndrome in the reference group was 71.15%.The efficacy of TCM syndrome in the test group was higher than that of the reference group,and the difference was statistically significant(P<0.05).(3)Before medication,there was no statistical difference in HbA 1 c,FBG and 2 h PBG in the two groups(P>0.05).After 8 weeks of medication,all glucose index in both groups had some decrease(P<0.05).FBG and HbA 1 c in the test group were lower than the reference group,The difference was statistically significant(P<0.05),and 2 h PBG and the reference group(P>0.05).(4)Before medication,there was no statistical difference in TG,TC,HDL-C and LDL-C(P>0.05).After 8 weeks of medication,all lipid indexes in both groups showed some improvement(P<0.05).HDL-C after 8 weeks was higher than that in the reference group,and the other lipid indexes were lower than the reference group,and the difference was statistically significant(P<0.05).(5)Before medication,there was no statistical difference in Cys-c,Scr and 24-UPRO in the two groups(P>0.05),After 8 weeks of medication,the renal impairment indicators improved in both groups(P<0.05).Cys-c,Scr,and 24-UPRO after 8 weeks of medication were lower than those in the reference group,with a statistically significant difference(P<0.05).(6)Neither group had any significant side effects or serious adverse reactions.The incidence of adverse reactions was 91.60.1%in the test group and 10.91%in the reference group.Comparing the two groups,the difference was not statistically significant(P>0.05).Conclusion:in the early clinical treatment of patients with DN using Jianpigushenhuoxue decoction with western medicine,can enlarge,better regulate blood sugar,lipid metabolism,help to improve kidney function injury,reduce proteinuria,relieve patients with TCM syndrome,improve clinical efficacy,and safety is good,has the value of further research application.
