Evaluation of white matter injury in people with circadian rhythm reversal based on multimodal MRIObjectives To explore the effect of circadian rhythm reversal on white matter injury based on multimodal magnetic resonance and provide auxiliary imaging basis for preventing and delaying white matter injury.Methods A total of 100 taxi drivers were recruited in this study.They were divided into white shift(n=50)and night shift(n=50)at 6:00 and 18:00.Collect demographic data,biochemical indicators,and head magnetic resonance image data,and match the age,blood pressure,blood lipids of the two groups of subjects.There are 34 people left in each group.①Determination of white matter hyperintensities(WMH):two experienced radiologists extract the MRI images of the participants by double-blind method,and evaluate the degree of WMH by Fazekas scale,And measure the volume of WMH through LST(Lesion Segmentation Toolbox)in SPM12 software;②Voxel-based morphometry(VBM)was used to analyze the different brain regions of white matter volume between the people with circadian rhythm inversion and the people with normal rhythm.③Diffusion tensor imaging(DTI)scanning was performed.DSI-Studio software was used to extract the fractional anisotropy(FA)of each ROI by taking the different brain regions analyzed by VBM as the regions of interest(ROI)Mean dispersion(MD),axial dispersion(AD)and radial dispersion(RD);The correlation analysis of WMH grade,WMH volume and FA,MD,AD and RD values of different brain regions was performed by independent sample t-test;The correlation between the characteristic value of white matter in different brain areas and the clinical disease and Fazekas rating was analyzed.Results① There were significant differences in the level and volume of white matter high signal between the normal circadian rhythm group and the reverse circadian rhythm group(p<0.05).② Compared with the normal group,the circadian rhythm inversion group showed a decrease in white matter volume in the right triangle inferior frontal gyrus(X=48,Y=36,Z=10.5),right fusiform gyrus(X=37.5,Y=-60,Z=-13.5),right middle temporal gyrus(X=-48,Y=-57,Z=0)and left middle temporal gyrus(X=-57,Y=-42,Z=-12).③The FA value,MD value,AD value and RD value of the left middle temporal gyrus were statistically different between the normal group and the inverted group(p<0.05),while the FA value,MD value,AD value and RD value of the right triangle inferior frontal gyrus,right fusiform gyrus and the right middle temporal gyrus were not statistically different between the normal group and the inverted group(p>0.05).④ The FA and RD values of the left middle temporal gyrus were significantly different between the hypertensive group and the non-hypertensive group in the population with circadian rhythm reversal(p<0.05).There was no significant difference in FA,MD,AD and RD between the diabetes group and the diabetes free group(p>0.05).There was a statistical difference in FA value between hyperlipidemia group and non-hyperlipidemia group(p<0.05).⑤ There was no correlation between FA value of left middle temporal gyrus and Fazekas rating of PWMH of lateral ventricle,DWMH of deep brain and WMH of whole brain(p>0.05).The MD value was positively correlated with the Fazekas rating of DWMH(r=0.42,p=0.017),but not with the Fazekas rating of PWMH in the lateral ventricle and WMH in the whole brain(p>0.05).The AD value was positively correlated with the Fazekas rating of WMH in the whole brain(r=0.32,p=0.022),but not with the Fazekas rating of PWMH in the measured ventricle and deep WMH(p>0.05).RD value was positively correlated with Fazekas rating of deep DWMH(r=0.374,p=0.035),but not with Fazekas rating of PWMH in lateral ventricle and WMH in whole brain(p>0.05).Conclusions① The white matter damage of people with circadian rhythm reversal was significantly higher than that of people with normal circadian rhythm.② Patients with hypertension and hyperlipidemia are more likely to cause white matter damage in people with circadian rhythm reversal.③ The white matter damage of people with circadian rhythm reversal mainly concentrated in the left middle temporal gyrus.
