Objective: To study the efficacy and safety of prophylactic hyperthermic intraperitoneal chemotherapy(P-HIPEC)for locally advanced gastric cancer after radical gastric cancer surgery.Methods: Data were retrospectively collected from gastric cancer patients at our hospital between 2018-2019,and 96 patients with pathological stage T3-4Nx M0 and who underwent D2 radical surgery for gastric cancer were included in the study and divided into HIPEC group(n=44)and control group(n=52)according to whether HIPEC was performed after radical surgery for gastric cancer.The incidence of complications,recurrence and survival rates,and risk factors for survival in both groups were analyzed.Results: The baseline data was not statistically different between the two groups and was comparable.There was no statistically difference in the 1-year postoperative survival rate between the two groups(84.1% vs 84.6%,P=0.958),and the 3-year postoperative survival rate of gastric cancer patients in the HIPEC group was higher than that of the control group,but the difference was not statistically significant(59.1% vs 51.9%,P=0.800).The univariate and multifactorial Cox regression analysis revealed that age,preoperative tumor marker levels,and surgical method were independent risk factors for the overall survival of gastric cancer patients after surgery.the recurrence rates of patients in the HIPEC group at 1,2,and 3 years were lower than those in the control group,but there was no statistical difference in the recurrence rates between the two groups.There was no significant difference between the two groups in the incidence of complications such as gastrointestinal anastomotic leakage(P>0.05),ileus(2.3% vs 1.9%,P>0.05),wound infection(4.5% vs 5.8%,P>0.05),and pulmonary infection(13.6% vs 13.5%,P>0.05).Conclusion:1.Prophylactic hyperthermic intraperitoneal chemotherapy does not increase the incidence of various postoperative complications in gastric cancer and is a safe and feasible treatment.2.Prophylactic hyperthermic intraperitoneal chemotherapy failed to significantly improve three-year postoperative survival in patients with advanced gastric cancer.3.Prophylactic hyperthermic intraperitoneal chemotherapy failed to significantly reduce the postoperative recurrence rate in patients with advanced gastric cancer.
