Objective:To compare the effects of inflation and methylene blue leaktesting on anastomotic complications in patients with radical gastrectomy for gastric cancer. Methods:Clinical case data of 381 patients undergoing radical gastrectomy for gastric cancerin Affiliated Nanchong Central Hospital of North Sichuan Medical College from September 2017 to September 2021 were retrospectively analyzed.Using the propensity score matching method, patient gender, age, body mass index(BMI),abdominal surgery history, complications, esophageal and gastric junction cancer, gastric gastrectomy, tumor markers and TNM pathological status were used as matching indicators, and 79 patients were included in each group(IOLT and NIOLT).The IOLT group performed anastomosis leak testing introperatively.NTOLT group did not perform leak testing introperatively.Clinical data, intraoperative situation, hospitalization time, hospitalization cost, anastomotic fistula incidence, and postoperative complications of patients were collected. Results:After propensity score matching, active anastomotic bleeding was found in 2 patients in IOLT group through gastroscopy, and incomplete anastomosis was found in 5 patients in IOLT group. Both patients successfully stopped bleeding after increased suture,There was no postoperative anastomotic fistula occurred. The operative time was longer in IOLT group than which in NIOLT group(P<0.05);one patient(1.3%)in IOLT group compared with patients in NIOLT group(10.13%),(P<0.05);postoperative hospitalization of patients with anastomosis was 38 days, which was statistically significant(P<0.05).Conclusion:Methylene Intraoperative leak teating can effectively reduce the occurrence of anastomotic leakage. The intraoperative leakage measurement technique combined with gastroscopy is recommended for radical gastrectomy for patients with gastric cancer.
