BackgroundIn recent years,overweight and obesity have become a major global public health problem.As is well known,obesity is an independent predictor of difficulty in mask ventilation and intubation,and is also associated with the occurrence of obstructive sleep apnea and obesity hypopnea syndrome,which affect patients’perioperative recovery and lead to upper respiratory tract obstruction,hypoxia,and postoperative pulmonary complications.Therefore,in order to ensure the safety and comfort of obese patients,it is necessary to quickly and reliably reverse neuromuscular blockade at the end of the surgery.ObjectiveTo compare the efficacy and safety of sugammadex sodium in reversing rocuronium-induced neuromuscular blockade in obese patients undergoing surgery.MethodsPatients undergoing elective laparoscopic surgery under general anesthesia in Jinan Central Hospital from April 2022 to January 2023 were randomly divided into experimental group and control group.The patients in the experimental group were given sugammadex sodium injection according to their lean bodyweight,and muscle relaxation monitoring continues until the patient leaves the operating room.The recovery time of TOFr to 0.9 was recorded.Extubation time;PACU stay time;Systolic blood pressure at 1 minute before administration,1 minute,5 minutes,15 minutes and 30 minutes after administration;Occurrence of adverse reactions(sinus bradycardia and other arrhythmias,allergic reaction,pain,dizziness,fever,nausea and vomiting,respiratory depression,restlessness,PPCS,etc.).Results1.Analysis of general data(Age,height,gender,abdominal circumference,total weight,body mass index,defatted weight)between the two groups of patients showed no statistically significant differences(P>0.05).2.There was no statistically significant difference in the dosage,administration temperature,surgical time,extubation time,and hospital stay of rocuronium between the two groups of patients(P>0.05).3.The comparison of sugammadex usage doses between the two groups of patients showed that the control group had a significantly higher dosage than the experimental group,and the results showed significant statistical significance(P<0.01).4.There was no statistically significant difference in the vital sign values between the two groups of patients at baseline T1,1 minute before administration,and 1 minute,5 minutes,15 minutes,and 30 minutes after administration(P>0.05).5.The comparison of the recovery time of TOFr>0.9 between the two groups of patients after applying sugammadex showed that the control group was significantly less than the experimental group,with significant statistical significance(P<0.01).Conclusion1.Both LBW and TBW doses of table sugammadex can effectively reverse the moderate NMB induced by rocuronium,but the LBW dose of table sugammadex significantly decreases.2.The defatted body weight dose significantly prolonged the time of reversing rocuronium-induced moderate neuromuscular blockade.
