| Objective This paper aims to explore the application of multiparameter of dual-layer spectral detector CT imaging in pathological classification of lung cancer.Methods The data of 101 patients with lung cancer diagnosed by pathological classification were collected.Of the 101 cases,there were 31 cases of small cell lung cancer,33 cases of lung squamous cell carcinoma and 37 cases of lung adenocarcinoma(all invasive lung adenocarcinoma).The dual layer spectral detector CT was used to scan and analyze the results of patients.The SBI data packets of all cases were uploaded to the nebula workstation.,and the analysis software was used to measure the multiparameters of images and record the data.The normalized iodine concentration and effective atomic number of ROI of all lesions were measured,the CT values of 40keV and 90 keV were recorded,and the slope of energy spectrum curve between 40keV and 90 keV was calculated.The normalized iodine concentration,effective atomic number and slope of energy spectrum curve were analyzed by Nonparametic rank sum test,and the Bonferroni method was used for intra-group comparison.The ROC curve was drawn,the diagnostic efficiency of each index was analyzed,and the optimal threshold was selected based on the Youden index.Results The results showed that there was no significant difference in age among different histopathological classification of lung cancer.The results of Nonparametic rank sum test showed that the normalized iodine concentration of small cell lung cancer group,squamous cell carcinoma group and adenocarcinoma group all showed an upward trend.There was no significant difference in normalized iodine concentration among the three groups in arterial phase(p>0.05).On the contrary,in the venous phase and delayed phase,there were significant differences in normalized iodine concentration among the three groups.The effective atomic number and the slope of the spectral curve in arterial phase,venous phase and delayed phase of the small cell lung cancer group,squamous cell carcinoma group and adenocarcinoma group all showed an upward trend,and the differences among groups were statistically significant(p<0.05).The result of Bonferroni method showed that the effective atomic number of small cell lung cancer group and the slope of the spectral curve were lower than those in adenocarcinoma group,and the differences were statistically significant(p<0.05).the normalized iodine concentrations between the small cell carcinoma group and the adenocarcinoma group were statistically significant in the venous phase and the delayed phase(p<0.05),and the normalized iodine concentrations,effective atomic number and slope of spectrum curve in the squamous cell carcinoma group and the adenocarcinoma group were statistically significant in the venous phase and the delayed phase(p<0.05).According to the calculation results of ROC curve,all parameters had certain diagnostic efficiency(AUC>0.5),some of which had high sensitivity and specificity.Conclusion In summary,spectral CT parameters in arterial phase,venous phase and delayed phase have certain clinical significance in the differentiating small cell lung cancer group,lung squamous cell carcinoma and lung adenocarcinoma. |
