Purpose 1.To explore the correlation between temporal resolving power of the auditory system and speech discrimination abilities under noise.2.To explore the effect of ageing and different degrees of hearing loss on the temporal resolving power of the auditory system of the elderly.Methods 30 young people with normal hearing were selected as the control group,30 elderly people with normal hearing,mild hearing loss,moderate hearing loss,moderate to severe hearing loss,and 20 elderly people wearing hearing Aids over the age of 60 were selected as the experimental group.Temporal resolving power of the auditory system(Gaps in Noise GIN)test and Mandarin quick speech-in-noise test(M-QuickSIN)were respectively carried out.The indices used were Gap Detection Threshold(GDT)and M-QuickSIN signal to noise loss(SNR loss).Statistical analysis was performed using SPSS 25.0.Results A total of 170 subjects who met the inclusion criteria were included in this study,including 30 normal hearing youth in the control group.In the experimental group,finally,20 elderly people in the hearing aid-wearing group finally met the inclusion criteria,and 30 in the other groups.The specific study results are as follows:1.Comparison of the correlation between the GDT and the M-QuickSIN SNR loss among the groups:There was no significant correlation between GDT and M-QuickSIN SNR loss(r=0.287,P=0.124>0.05)in normal hearing youth.There was no significant correlation between GDT and M-QuickSIN SNR loss(r=0.235,P=0.210>0.05)in normal hearing in the elderly people.There was no significant correlation between the GDT and M-QuickSIN SNR loss in the elderly with mild hearing loss(r=-0.707,P=0.714>0.05).However,there is a statistically significant correlation between the GDT and M-QuickSIN SNR loss in the elderly with moderate hearing loss(r=0.856,P=0.000<0.05).There is a statistically significant correlation between the GDT and M-QuickSIN SNR loss in the elderly with moderate to severe hearing loss(r=0.514,P=0.004<0.05).There is a statistically significant correlation between the GDT and M-QuickSIN SNR loss with same elderly people before and after wearing a hearing AIDS(before=0.719,P=0.000<0.05,after=0.888,P=0.000<0.05).And the average value of the GDT in the elderly after wearing hearing AIDS(10.08±3.53ms)close to the level of moderate hearing loss elderly(11.15±2.91ms),meanwhile,the average value of M-QuickSIN SNR loss(5.85±1.39ms)close to the level of moderate hearing loss(5.722±1.35ms).There is a statistically significant correlation between the GDT value and M-QuickSIN SNR lossamong all groups(r=0.825,P=0.000<0.05).2.Comparison in terms of age:There is a statistically significant about the GDT between the young people with normal hearing and the elderly with normal hearing(P=0.001<0.05).3.Comparison of the degree of hearing loss in the elderly:There was no statistically significant in GDT between normal hearing and mild hearing loss(P=0.863>0.05),GDT of the elderly with normal hearing,moderate hearing loss,and moderate to severe hearing loss are statistically significant(P=0.000<0.05).4.Comparison of the elderly with moderate to severe hearing loss after wearing hearing Aids and other groups:The GDT of the elderly after wearing hearing Aids is greater than that of the young people with normal hearing,the elderly with normal hearing and mild hearing loss,and the results are statistically significant(P=0.000<0.05).However,the GDT between the elderly after wearing Aids and the elderly with moderate hearing loss was not statistically significant(P=0.144>0.05).The GDT of the elderly after wearing hearing Aids is better than that of the elderly with moderate to severe hearing loss,and there is a statistical significance(P=0.043<0.05).There is a statistical significance of GDT before and after wearing hearing Aids in the same elderly(P=0.000<0.05).Conclusion ①There is no significant correlation between the GDT and the M-QuickSIN SNR loss of young people with normal hearing,the elderly with normal hearing and the elderly with mild hearing loss,but there is a clear correlation between moderate and above hearing loss,before and after hearing Aids wearing.Moreover,the average of the GDT and M-QuickSIN SNR lossafter wearing the hearing Aids both reached the level of moderate hearing loss in the elderly,the results suggest the GDT correlates with M-QuickSIN SNR loss in a certain degree of hearing loss,the higher GDT,the worse M-QuickSIN SNR loss.After long-term wearing of hearing Aids,both GDT and M-QuickSIN SNR lossreached the level of the elderly with moderate hearing loss,it is suggested that the wearing of hearing Aids can reduce the GDT and improve M-QuickSIN SNR loss.When the subject cannot perform speech audiometry,GIN test can be considered for clinical reference.②The GDT of the elderly with normal hearing is higher than that of young people with normal hearing,and the results suggest that increasing age has an impact on the GDT.③The GDT of the elderly with normal hearing and mild hearing loss is close,but the GDT of the elderly with moderate or higher hearing loss is significantly higher than that of the elderly with normal hearing,indicating that the degree of hearing loss affects the GDT of the elderly,and the heavier hearing loss,the larger GDT.④The GDT of the elderly with moderate to severe hearing loss after wearing hearing Aids is close to that of the elderly with moderate hearing loss,and is better than that of the elderly with moderate to severe hearing loss,indicating that wearing hearing Aids can affect the GDT and reduce it. |
