Objective 1.To investigate the effect of PLR and Cys C on the severity of obstructive sleep apnea-hypopnea syndrome(OSAHS)and whether it can aggravate vascular endothelial dysfunction in patients with OSAHS.2.To analyze the influencing factors of vascular endothelial function in OSAHS,and provide reference value for prevention and treatment of OSAHS complications.Methods We selected 156 patients from Qinghai Hospital(2020.01-2021.12),and selected them strictly according to the criteria of inclusion and exclusion,to improve brachial artery color doppler ultrasound and measure brachial artery vasodilation function(FMD)and polysomnography(PSG),according to the guideline of multidisciplinary diagnosis and treatment of adult obstructive sleep Apnea(2018edition),Apnea hypopnea index was divided into four groups: control group(N=34,AHI≤5/h);OSAHS mild group(n=40,5 times AHI<15 times/h);OSAHS moderate group(N=42,15 times/h AHI < 30 times/h);OSAHS severe group(N=40,AHI≥30times/h).SPSS25.0 software was used to collect general data,blood biochemistry and examination data.Results the BMI level in moderate and severe OSAHS group was higher than that in control group and mild OSAHS group(p = 0.000),but there was no significant difference in age,sex,smoking history and drinking history(P > 0.05).the main index of inflammation: the level of Cys C in OSAHS group was higher than that in control group(p = 0.000),and the level of PLR had no significant difference(p =0.510).other related indexes: there were significant differences in RBC,HGB,TC,CR,UA,left and right IMT,FMD,ly%,diameter of brachial artery at rest and maximal dilatation in each group(p < 0.05).In addition,there was no significant difference in PLT,WBC,LY,MO,Ne%,Mo%,TG,UN etc.(P > 0.05).binary Logistic regression analysis showed that the level of Cys C,BMI,RBC,HGB,TC,CR,UA and right side IMT were the influencing factors of vascular endothelial dysfunction in patients with OSAHS(p < 0.05)However,Cys C,BMI and UA were the significant factors(p =0.000,0.002,0.024),OR values were 31.530,1.184,1.006,respectively,statistically significant.Spearman correlation analysis showed that Cys C,RBC,HGB,UA,left and right IMT,BMI were negatively correlated with FMD(rs =-0.451,-0.296,-0.280,-0.385,-0.213,-0.222,-0.399,p < 0.05),p LR,TC,TG,CR,age had no correlation with FMD(P > 0.05).the abnormal endothelium function of OSAHS was predicted by Cys C,HGB,RBC,BMI,PLT ROC curve.When predicting endothelium function of OSAHS by Cys C,HGB,RBC,BMI,there was statistical difference(p < 0.05),however,there was no significant difference between PLR and AUC in predicting the endothelial function of OSAHS(P > 0.05).when combined with BMI and Cys C,the AUC of Cys C + BMI increased to 0.871(p < 0.01).Conclusion 1.The level of Cys C is the risk factor of endothelial dysfunction in OSAHS patients.The higher the level of Cys C,the lower the FMD,the higher the risk of endothelial dysfunction PLR may not be a risk factor for endothelial function in OSAHS patients.2.BMI,RBC,HGB,UA,and IMT were found to affect the Endothelium function of OSAHS patients,and were correlated with the severity of OSAHS.3.Cysc combined with BMI,RBC,HGB,UA and IMT may have better predictive value for the severity of vascular endothelial function and prognosis of OSAHS patients.
