| Objectives This study summarizes the analysis of the risk factors of postpartum venous thromboembolism in puerperae,constructing a Quick Risk Evaluation Form.Be able to quickly and effectively assess risks,for the development of appropriate clinical treatment plans。Methods A total of 196 puerperae who delivered in the obstetrics department of Peking University Shenzhen Hospital from January 2012 to January 2019 were selected.According to whether there is postpartum VTE,we divided patients into two groups,gather information about maternal medical records.Analyze data using SPSS26.0,analysis of independent high-risk factors using chi-square test and analysis of variance,we used the logistic regression model to analyze the correlation between the independent high-risk factors,constructing a Quick Risk Evaluation Form.The Hosmer-Lameshaw test was used to assess the fit of the risk prediction model and the Quick Risk Evaluation Form.Using ROC curve and AUC to detect the predictive value of risk prediction model and the Quick Risk Evaluation Form,AUC<0.7 means low diagnostic accuracy,0.7-0.9 means moderate diagnostic accuracy,>0.9 means high diagnostic accuracy.Discrimination the Quick Risk Evaluation Form using t-test.Assess the sensitivity,specificity and Youden index of the Quick Risk Evaluation Form,P < 0.05 was considered statistically significant difference.At the same time,puerperae who are included in the study was used for preliminary verification with the Quick Risk Evaluation Form.Results There were statistically significant differences in number of fetuses,history of previous thrombosis,age,history of gestational hypertension,method of conception,family history(blood clots,diabetes,cardiovascular disease),admission BMI and mode of delivery between the VTE group and non-VTE group(P<0.05).There were no significant differences in parity,history of miscarriage,history of smoking,history of drinking,history of gestational diabetes,history of anemia during pregnancy,history of pre-pregnancy contraceptive use and postpartum hemorrhage(P>0.05).Combined with the latest guidelines at home and abroad,various clinical studies and logistic multivariate analysis results,constructing a Quick Risk Evaluation Form.Conclusion The AUC of this rapid assessment profile predicts postpartum complicated VTE of 0.953(>0.9),95% CI 为 0.925-0.981,the sensitivity was 87.2%,the specificity was 89.1%,and the Youden index was 76.3%,the Quick Risk Evaluation Form have better predictive value. |
