Objective1.To investigate the status of illness Perception, family support and self-perceived burden of diabetic retinopathy patients.2.To explore the influencing factors of self-perceived burden in diabetic retinopathy patients.3.To analyze the correlation between illness Perception, family support and self-perceived burden in diabetic retinopathy.4.By constructing the structural equation model of the three, illness perception on patients with diabetic retinopathy disease is through the family support function of self-burden,in order to provide some references for the implementation of nursing intervention to improve the mental health of patients with diabetic retinopathy, the quality of life.Methods1.This study was a cross-sectional study.416 patients with diabetic retinopathy who met the inclusion and exclusion criteria in the endocrinology department, ophthalmology clinic and chronic Disease management center of a hospital in Inner Mongolia from March 2021 to December 2021 were selected as the subjects.2.The survey tools include a self-designed general information questionnair-e,Chinese Illness Perception Questionnaire,Family Support Questionnaire and the Self-Perceived Burden Scale.3.Statistical analysis software SPSS 25.0 and AMOS 23.0 were used to process the data.Results1.A total of 422 questionnaires were collected in this study, including 6 invalid ones and 416 valid ones, with effective recovery of 98.58%.2.The self-perceived burden score of diabetic retinopathy patients was(29.20±7.41),which was at the level of mild burden.Physical burden(14.59±3.81)points,emotional burden(11.57±3.35)points,economic burden(3.04±1.10)points.3.The illness perception score of diabetic retinopathy was(49.16±10.89),which was in the middle level. The scores of cognitive dimension, emotional dimension and comprehension ability were(30.35±6.92),(12.11±4.06)and(6.71±2.12)respectively.The score of family support was(10.41±2.35),which was at the high level of family support.4.Pearson correlation analysis showed that:there was a positive correlation between illness perception and self-perceived burden, physical burden, economic burden and emotional burden(r>0,P<0.01).There was a positive correlation between family support and self-perceived burden, physical burden, economic burden and emotional burden(r>0,P<0.01).There was a positive correlation between cognitive dimension, emotional dimension, understanding ability dimension and self-perceived burden(r>0,P<0.05).5.Multiple regression analysis showed that:monthly family total income, illness perception and family support were the influencing factors of self-perceived burden of diabetic retinopathy patients, and the scores were statistically different(P<0.05).6.The results of structural equation model showed that:the fitness of struc-turalequation model of illness perception, family support and self-perceived b-urdenin diabetic retinopathy patients was better,,?~2/df=1.224,GFI=0.991,AGFI=0.997,NFI=0.985,CFI=0.997,RMSEA=0.023。7.The results of mediating effect test showed that illness perception played a part mediating effect between family support and self-perceived burden in diabetic retinopathy patients, and the mediating effect is 87.20%.Conclusion1.The illness perception of diabetic retinopathy patients is at a medium level, family support is at a high level, and self-perceived burden is at a low level.2.Personal monthly income, monthly household total income, diabetes education, treatment type, illness perception and family support were the influencing factors of self-perceived burden of diabetic retinopathy patients.3.The illness perception in patients with diabetic retinopathy can directly affect the self-perceived burden.Family support can directly affect self-perceived burden in diabetic retinopathy patients, and family support can indirectly affect self-perceived burden through illness perception, and illness perception plays a partial mediating effect between the two.Prompt nursing staff can from health education, cognitive behavior intervention, emotional support and other aspects,to improve the patients with diabetic retinopathy disease awareness to reduce the burden of self feeling, also can by family members of patients with diabetic retinopathy mission to create a good family atmosphere, reduce the burden of patients self feelings from different angles. |
