| Objective: To investigate the clinical significance of peripheral blood fibrinogen to albumin ratio(FAR)in patients with isolated coronary artery ectasia(ICAE).Methods: This article is a retrospective analysis.From November 2015 to January 2022,7020 patients were admitted to the Second Department of Cardiology,the Second Affiliated Hospital of Hebei Medical University due to chest pain,chest tightness and other reasons and underwent coronary angiography were selected.A total of 150 patients with arterial ectasia(CAE),including 64 patients with simple coronary artery dilatation,were selected from the patients who underwent coronary angiography at the same time,with the normal coronary artery(normal coronary artery,NCA)150 cases.To avoid the influence of clinical confounding factors such as gender,age,and BMI,after 1:1 propensity score matching(PSM),the final number of cases obtained was: isolated coronary ectasia(ICAE)group 53 People,53 people in the normal coronary artery(NCA)group.The demographic data,blood routine,blood biochemistry,echocardiographic indexes and comorbidities and other clinical data of the patients were counted and FAR was calculated.The correlation between the above indicators in the ICAE group and the NCA group was analyzed;the independent predictors of pure ICAE were analyzed by multivariate analysis..Results: A total of 53 patients with ICAE and 53 patients with NCA were included in this study.Demographic data,BMI,smoking history,hypertension,diabetes mellitus,hyperlipidemia,white blood cells,neutrophil count,lymba cell count,haemoglobin,platelet count,high-sensitivity Creactive protein,total bilirubin in both groups,direct bilirubin,indirect bilirubin,aspartate aminotransferase,alanine aminotransferase,alkaline phosphatase,serum creatinine,total cholesterol,triglycerides,high-density lipoprotein,low-density lipoprotein,Glycated haemoglobin,left ventricular end-diastolic diameter,left ventricular ejection fraction,E/A,E/e’,e’ and other factors were not significantly different.Compared with the normal coronary group,the albumin(ALB)of the pure CAE group(40.68±3.17 vs 43.18±6.72,P=0.017),the inner diameter of the ascending aorta(AAD)(33.49 ±3.36 vs 32.08±3.85,p=0.046),fibrin Pro/albumin(FAR)(7.39±1.61 vs 6.52±1.40,P=0.004)had significant statistical difference.Multivariate analysis showed that FAR(OR: 1.422,95%CI: 1.049-1.926,P=0.023),AAD(OR=1.143,95%CI: 1.015-1.288,P=0.027)were independent of isolated coronary ectasia risk factors.ROC curve analysis: fibrinogen/albumin AUC was 0.651(95%CI: 0.548-0.755,P=0.007),the best cut-off value was 7.67,the sensitivity was 41.5%,and the specificity was 84.9%.Conclusion: The FAR of patients with ICAE is higher than that of patients with normal coronary arteries.FAR may be a risk factor for the occurrence of ICAE and has certain diagnostic value for the occurrence of ICAE,but its diagnostic sensitivity is low. |
