Objective:To investigate the clinical value of brain natriuretic peptide(BNP),acute physiology and chronic health evaluation(APACHE)Ⅱscore and lung ultrasound(LUS)score in severe pneumonia patients,so as to provide a reference for clinical treatment.Methods:The 89 severe pneumonia patients admitted to our hospital from January 2019 to December 2021 were divided into two groups: death(23)and survival(66)based on the 28-day survival outcomes after 9 days in the treatment of Critical care Medicine(ICU).Record the BNP values,APACHEⅡ score and LUS scores on days 1,3,5,7 and9 after ICU admission,compare the differences in BNP level,APACHEⅡ score and LUS score,respectively,and analyze the correlation between BNP level and APACHEⅡ score and LUS score.Results:Compared with day 1,BNP levels,APACHEⅡ scores,and LUS scores were higher on day 3,but on days 5,7,and 9 they all decreased,while BNP levels,APACHEⅡ scores,and LUS scores increased on days 1,3,5,7,and 9 in the death group.Univariate analysis found that the BNP level,APACHEⅡ score and LUS score were all statistically different from day 5(P<0.05).Pairwise correlation analysis of the three found that BNP level and APACHEⅡ score were positively related on days1,3,5,7 and 9(r=0.608,0.613,0.834,0.863,0.892,P<0.05).Among them,BNP levels were correlated moderately with APACHEⅡ scores on survival on days 1 and 3,and strongly on days 5,7 and 9.Both the LUS scores and the APACHEⅡ scores were positively associated on days 1,3,5,7,and 9(r=0.675,0.577,0.822,0.878,and 0.87,P<0.05).Among them,a moderate intensity correlation between LUS score and APACHEⅡ score occurred on the survival and death groups on days 1 and 3,and a strong correlation occurred on days 5,7,and 9.Both the BNP levels and the LUS scores were positively associated on days 1,3,5,7,and 9(r=0.599,0.584,0.844,0.888,0.889,P <0.05).Among them,BNP levels and LUS ratings were moderate on days 1 and 3,and were strongly correlated on days 5,7 and9.The combined multifactor analysis of the three factors found that there was no significant difference in the combined analysis of BNP level,APACHEⅡ score,and LUS score between the two patient groups(P> 0.05),as in the univariate analysis on day 1.However,the combined analysis varied significantly without differences in the three univariate analysis on day 3(P <0.05)Conclusion:The BNP level,the APACHE Ⅱ score and the combined LUS score have a certain guiding role in the disease judgment and prognosis prediction of patients with severe pneumonia.
