| Objective:To understand the occurrence and treatment of anemia in peritoneal dialysis(PD)patients in Shanxi Province,and to study its risk factors.Methods:The case data of PD patients reported by the Chinese national renal data System in Shanxi Province in 2020 were collected,including basic information,dialysis status,drug use and laboratory indicators.and analyze the data.Results:(1)In 2020,the average Hb level of PD patients in Shanxi Province was 104.9±18.3g/L,and the overall Hb compliance rate was 40.2%.Erythropoiesis-stimulating agent use was 90.8%,iron use was 24.6%,and 98.3% of patients used oral iron.Gender stratification showed that the Hb compliance rate of men was higher than that of women,and age stratification showed that with the increase of age,the Hb compliance rate showed an increasing trend.(2)Compared with the Hb-unreached group,the Hb-attainment group had higher proportion of males,average age,occupational stability,transferrin saturation,serum calcium and serum albumin;dialysis age,weekly ESAs dose,continuous ambulation The daily dialysate dose,ferritin,serum creatinine and serum phosphorus were lower in Continuous ambulatory peritoneal dialysis(CAPD)patients(all P<0.05).(3)Multivariate logistic regression analysis showed that young age,high parathyroid hormone and low serum albumin levels were the risk factors for Hb failure to reach the target.Conclusion:The overall compliance rate of Hb in peritoneal dialysis patients in Shanxi province(Hb≥110g/L)is 40.2%,which is still a gap with developed areas.Paying attention to the diagnosis and treatment of anemia in young patients,reducing the level of parathyroid hormone and improving the nutritional level may improve the anemia of PD patients in our province.Objective:To investigate the epidemiology changes of patients with initial hemodialysis in Shanxi province from 2010 to 2020,including the incidence,the composition of the primary disease,the vascular access,and the control of various complications of the initial hemodialysis,so as to provide support for the quality control of blood purification.Methods:The case data of hemodialysis patients directly reported on the Internet from all medical institutions carrying out blood purification treatment in Shanxi Province from 2010 to 2020 were retrospectively analyzed.The collection of various relevant data is completed through the Chinese national renal data System(CNRDS).Results:The annual incidence of new hemodialysis in Shanxi Province from 2010 to 2020 was 46.62,53.44,62.61,62.93,42.98,36.63,45.23,46.23,46.3,64.3,and 83.03 per million population,respectively,2010-2013 and 2017-2020 are two periods of rapid growth.the number of new hemodialysis cases and the annual incidence of initial hemodialysis from 2010 to 2020 in the male patients were higher than those in the female patients men were higher than those in women.From 2010 to 2020,the main age groups of patients with endstage renal disease in Shanxi Province who started dialysis changed from 40-50 years and 50-60 years old to 50-60 years old and 60-70 years old.The top three primary causes of hemodialysis patients were chronic glomerulonephritis,diabetic nephropathy,and hypertensive renal damage,among which chronic glomerulonephritis has dropped from 60.9% in 2010 to 36.6% in 2020,while diabetic nephropathy and hypertensive renal damage from 23.3% and 7.9% in 2010 to 36.3% and 13.3% in 2020,respectively.The use of autogenous arteriovenous fistulas for hemodialysis increased from 43.3% to 50.8% in 2018,while the use of temporary central venous catheterization dropped from 44.1% in 2010 to 39.3% in 2018.From 2012 to 2020,the control rate of hemoglobin of new hemodialysis patients was not improved significantly,fluctuating from 15.1% to 21.9%,while the serum albumin control rate increased from 51.9% to 57.2%.The systolic blood pressure control rate was between 70.8 and 76%.The serum calcium control rate from 39% in 2012 to 46.7% in 2010,and the serum phosphorus control rate was between 39.8% and 47.3%.The control rate of serum intact parathyroid hormone was less than 60% without apparent fluctuation.Conclusion:(1)From 2010 to 2020,the annual incidence of hemodialysis in Shanxi Province increased in fluctuation,peaking in 2020.(2)The age of hemodialysis patients in Shanxi Province has changed from young to middle-aged.(3)The etiology of hemodialysis changed from primary kidney disease to secondary kidney disease.(4)The utilization rate of first-time dialysis arteriovenous fistula was increasing,and the utilization rate of temporary central venous catheter was decreasing.(5)The control compliance rate of complications when entering dialysis has improved,but it still needs to be improved. |
