| Purpose:Explore the right side of the small incision line of mitral valve plasty in the treatment of mitral valve disease with traditional open heart surgery keratoplasty for patients disease curative effect comparison and analysis,clear the advantages and disadvantages of small incision line valve repair,understand the small incision surgery in safety,efficacy,postoperative complications,and so on and so forth,which provide more safe and effective for patients with the best treatment plan to provide certain reference significance.Methods:Retrospective analysis was performed on the data of 70 patients who underwent mitral valve plasty in the department of cardiac Macrovascular Surgery of our hospital from January 2019 to December 2021,including 50 patients who underwent right small orifice plasty as observation group(group A)and 29 patients who underwent traditional thoracotomy for mitral valve plasty as control group(group B).The operative time,CPB time,aorta occlusion time and intraoperative blood loss of the two groups were compared and analyzed.Comparative analysis of mean mechanical ventilation time,mean ICU stay,24 h postoperative drainage volume,postoperative blood transfusion volume,average length of hospital stay,duration of postoperative vasoactive drug use,incidence of postoperative complications,and postoperative cardiac function.Results:There was no perioperative death in the observation group and the control group,and no intraoperative conversion to thoracotomy was found in the observation group.All patients recovered after surgery and were discharged.1)Comparison and analysis of preoperative basic data: there were no significant differences in age,sex,height,weight,underlying diseases,left atrial diameter and left ventricular end-diastolic diameter between the two groups(P > 0.05);2)The operative time between the two groups(group A =255.36±42.88,group B=223.31±25.54)was significantly different(P < 0.05).The cardiopulmonary bypass time(120.58±33.83)and aorta blocking time(84.12±24.87)in the observation group were significantly longer than that in the control group(103.10±21.43)and aorta blocking time(70.83±18.48),the differences were statistically significant(P <0.05).Intraoperative blood loss in the observation group(402.00±210.43)was significantly lower than that in the control group(610.34±181.94),and the difference was statistically significant(P < 0.05).3)the average observation group of patients in postoperative mechanical ventilation time,average ICU stay time,24 h after surgery,postoperative blood transfusion amount,drain the average hospitalization days after surgery,postoperative vascular active drug use time are shorter than control group in time,there is statistical significance difference(P < 0.05),in terms of postoperative complications,observation group of pleural effusion in 1 case,In the control group,there were 2cases of cardiac insufficiency,2 cases of pleural effusion and 2 cases of poor incision healing.The incidence of total complications was 20.70%,and the difference of postoperative cardiac function was statistically significant(P < 0.05).Conclusion:Right Mini-thoracotomy is sigifantive better than traditional open heart surgery in intraoperative blood loss,postoperative mechanical ventilation on average time,average ICU stay time,24 h after surgery,postoperative blood transfusion amount,drain the average hospitalization days after surgery,postoperative vascular active drug use time significantly superior to traditional open heart surgery,and on the postoperative complications than traditional surgery without rising trend,The operative time is slightly longer,and compared with the traditional operation,the surgical incision is smaller,the effect on the chest wall and sternum is less,and the recovery is faster.Under the condition of strictly grasping the indications,mitral valve plasty through the right small incision has more obvious advantages. |
