Objective: To investigate the short-term clinical efficacy of prostatic arterial embolization(PAE)in the treatment of benign prostatic hyperplasia(BPH)and to analyze the correlation between PAE and prostate-specific antigen(PSA)changes within24 hours after operation.Methods: A retrospective analysis was conducted to collect 62 BPH patients from Suining Central Hospital from July 2019 to August 2021 according to inclusion and exclusion criteria.Patients receiving PAE were treated as the observation group(PAE group: 17 cases),and patients receiving TURP were treated as the control group(TURP group: 45 cases).To collect data,including patient age,basic diseases,duration,postoperative time placing a urinary catheter,length of hospital stay,the international prostate symptom score(IPSS score)before and after operation,the quality of life score(Qo L score),residual urine volume(PVR),prostatic volume(PV),the cause of postoperative adverse reactions,and preoperative PAE group of PSA in 24 hours after surgery.T test,Mann-Whitney U test,chi-square test or Fisher’s exact probability method were used for analysis according to data types and characteristics.Spearman correlation analysis was used to evaluate the correlation between PSA increase multiple and IPSS score improvement value within 24 h after PAE treatment.Results: In the PAE group,17 patients were successfully treated with PAE(100%),including 16(94.1%)bilateral embolization and 1(5.9%)unilateral embolization.There were 45 patients in the TURP group,and all patients(100%)successfully completed surgery.The median length of hospital stay was 9.0(IQR:8.00,9.50)days in the PAE group and 12.00(IQR:10.00,15.00)days in the TURP group.The median length of hospital stay was shorter in the PAE group than in the TURP group(P < 0.05).IPSS score,Qo L score,PV,PVR and other indicators were compared between the two groups before surgery and 1 month and 3 months after surgery0.05).There were no significant differences in IPSS score,Qo L score,PV and PVR between the two groups 3 months after surgery(ALL P > 0.05).PV of TURP group was lower than that of PAE(P <0.05).Adverse reactions of varying degrees occurred in 6 patients(35.3%)in the PAE group and 24 patients(53.3%)in the TURP group.After examination,the incidence of postoperative adverse reactions in the PAE group was lower than that in the TURP group(P < 0.05).The correlation analysis and test showed that there was a statistically significant positive correlation between the change multiple of PSA and the change value of IPSS within 24 hours after PAE treatment(P=0.02,r=0.54).Conclusion:(1)The clinical efficacy of PAE and TURP for BPH is similar,and it can be used as a complementary treatment modality.(2)The PSA increased significantly within 24 hours after PAE treatment,and the patients with more increases,the better the recent efficacy,so it may be used as one of the evaluation indicators of the recent clinical efficacy. |
