| Objective:Objective To investigate the characteristics of adiponectin(APN)and leptin(LEP)levels in patients with diabetic limb arterial occlusion(DAO)and their correlation with the TCM syndromes of DAO.Methods:In study 1,60 DAO patients who met the inclusion criteria in the Peripheral vascular Department of Shandong University of Traditional Chinese Medicine Affiliated Hospital were selected as the observation group,and 30 physical examination subjects were selected as the control group.General information{gender,age,history of hypertension,body mass index(BMI)},hematological indicators {HbAlc,triglyceride(TG),total cholesterol(TC),high density lipoprotein(HDL),low density lipoprotein(LDL),APN,LEP} of the two groups were collected.The differences of above indexes between the two groups were compared.The observation group was stratified according to gender,age,course of disease,clinical stage,history of hypertension and BMI.Different statistical methods were used to further explore the correlation between detection indicators and their diagnostic value to DAO.In study 2,60 PATIENTS with DAO that met the criteria of TCM syndrome differentiation were selected,including 30 patients with blood stasis type and 3 0 patients with damp-heat type,to explore the differences of the above indicators in different TCM syndrome types and the correlation between the indicators and TCM syndrome types.Results:Study 1:1.Compared with the control group,the proportion of DAO patients with hypertension history was higher(P<0.01);Age,BMI,HbAlc and LEP levels were higher(P<0.01).HDL level was lower(P<0.05).The level of APN was lower(P<0.01).2.Compared with female patients,male DAO patients had higher APN level(P<0.05);With the increase of age,THE LEVEL of APN in DAO patients gradually decreased(P<0.05),and the level of LEP gradually increased(P<0.01).With the increase of disease course,LEP level of DAO patients increased gradually(P<0.01).With the development of clinical stage,LEP level of DAO patients increased gradually(P<0.01).The APN level of obese DAO patients was lower than that of non-obese patients(P<0.05),and the LEP level was higher than that of non-obese DAO patients(P<0.01).3.Univariate binary Logistic regression analysis showed that LEP was risk factor for DAO,APN was protective factor.4.The areas under ROC curve of APN and LEP were 0.596 and 0.736,respectively.Spearman correlation analysis showed that age,course of disease,BMI,HbAlc,TG,LEP were negatively correlated with APN.Age,course of disease,BMI and HbAlc were positively correlated with LEP,while APN and HDL were negatively correlated with LEP.Study 2:Compared with blood stasis DAO patients,damp-heat wound-type patients were older(P<0.01),had longer course of disease(P<0.01),and had less history of hypertension(P<0.01);BMI,HbAlc and LEP levels were higher(P<0.01),while HDL levels were lower(P<0.05).Conclusion:APN and LEP are closely related to the occurrence and development of DAO.The expression levels of APN and LEP are different in different genders,ages,disease course,clinical stages,BMI and with or without history of hypertension,which have certain reference value for clinical diagnosis and disease evaluation of DAO.LEP expression level is different in different syndrome types of DAO,which can be an effective supplement for DAO syndrome differentiation. |
