Objective The aim of this study was to systematically review and analyze the medical records of elderly patients with hypertension combined with chronic obstructive pulmonary disease,and to investigate the relationship between the distribution of their TCM evidence patterns and various factors by statistically analyzing the age,gender,smoking history,drinking history,disease duration,hypertension classification,comorbidities and other relevant factors and TCM evidence patterns and clinical indices of elderly patients with hypertension combined with chronic obstructive pulmonary disease,in order to provide a relevant reference basis for TCM evidence classification and clinical treatment of elderly patients with hypertension combined with chronic obstructive pulmonary disease,and to improve the survival quality of elderly patients.Methods 197 cases meeting the inclusion criteria were selected from inpatients of the Department of Geriatric Medicine and the Department of Cardiology of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from December 01,2019 to December 31,2021.Clinical data related to gender,age,duration of hypertension,duration of chronic obstructive pulmonary disease,history of smoking,history of alcohol consumption,clinical indicators,and TCM evidence type were collected for the studied subjects.The collected data were entered into the Excel system,and the data were statistically processed and analyzed by SPSS26.0 software.Results 1.Among the 197 cases included in this study,TCM evidence is most commonly confirmed by sputum stasis,accounting for 27.42% of the total number;2.Cerebrovascular disease in the center of the comorbid disease center accounts for the largest proportion,of which coronary atherosclerotic heart disease accounts for 64.5% of the total number,followed by cerebral infarction accounts for 45.2%;3.Of all cases,men account for 69%,women account for 31%,and the proportion of men and women is 2.2%.:1,there was no statistical significance of the sex difference between the syndromes(P>0.05);4.Among all the patients included in the study,the number of patients aged 70-79 was the largest,of which the average age of the patients with qi and yin deficiency was the largest,and there was a significant difference in the age comparison between the syndromes(P<0.01);5.There were significant differences between the symptoms of traditional Chinese medicine and hypertension and the course of COPD(P<0.01),of which qi and yin were both false and negative.6.There are differences between each TCM syndrome and smoking history,drinking history,and smoking index(P<0.05),among which there are the largest number of patients with smoking history and drinking history who have evidence of sputum stasis,and the average smoking index of qi and yin is significantly higher than that of other types;7.There are differences between various TCM syndromes and hypertension classification and risk stratification.It was statistically significant(P<0.05),of which hypertension grade 3 and very high-risk patients were concentrated in sputum stasis mutual evidence;8.between the syndromes,FBG,TG,TC,LDL-C,HDL-C,FIB,WBC,RBC,Hb,PLT,BNP were statistically significant(P<0.05);among them,in FBG and TG,the intraphlegm obstruction and sputum stasis mutual codification were significantly higher than those of the other four groups;in terms of TC and LDL-C,In terms of HDL-C,the mutual evidence of intra-sputum obstruction and sputum stasis was significantly higher than that of the other three groups;in terms of HDL-C,the mutual evidence of intra-sputum obstruction and sputum stasis was significantly lower than that of the other four groups;in terms of FIB,the means of mutual confirmation of sputum stasis,intramutum obstruction and qi deficiency and blood stasis were significantly higher than those of other three types;the mean values of WBC and PLT in the mutual evidence of sputum stasis were significantly higher than those of other types,and the average values of RBC and Hb of qi and yin deficiency and qi deficiency and blood stasis were significantly lower than those of other types9.According to logistic regression analysis,FBG,TG,HDL-C,FIB,WBC,RBC,Hb,PLT,and BNP are relevant factors in the classification of TCM types.Conclusion 1.Elderly patients with hypertension and COPD are still mainly based on the virtual standard,the deficiency is mainly qi deficiency and yin deficiency,and the standard reality is mainly sputum turbidity and blood stasis.2.In traditional Chinese medicine,sputum stasis is the most common,and the distribution between the symptoms is not significantly related to sex,INR,D-D,NE,CRP,and c Tn I.The average age of patients with qi and yin deficiency is the largest,which may be because the patient’s lung function is significantly reduced with age and course,coupled with the influence of smoking and other factors,the qi and yin deficiency is more obvious.3.FBG,TG,TC,LDL-C,HDL-C,FIB,WBC,RBC,Hb,PLT,BNP are closely related factors in the classification of TCM syndromes,and FBG,TG,HDL-C,FIB,WBC,RBC,HB,HB,PLT,BNP have significant differences between the syndromes,which can be used as a reference for the objective indicators of TCM dialectical treatment.
