ObjectiveThe purpose of this study is to find the risk factors of post-stroke cognitive impairment and the indicators that can reflect the post-stroke cognitive status through the analysis of the clinical data and quantitative electroencephalography(qEEG)data of patients with post-stroke cognitive impairment,so as to provide a theoretical basis for the prevention,evaluation and prognosis of cognitive impairment.MethodThis study included 40 patients with cognitive impairment after acute ischemic stroke treated in the Department of Neurology of Shizhong District People’s Hospital of Shandong Provincial Hospital(Group)from September 2020 to December 2021 and 42 healthy volunteers examined in the same hospital during the same period.General condition,laboratory examination,assessment scales(Mini-mental State Examination,MMSE,National Institute of Health stroke scale,NIHSS,Modified Rankin Scale,mRS,Hamilton Anxiety Scale,HAMA and Hamilton Depression Scale,HAMD)and qEEG were collected.The data were divided into groups,calculated and processed,and then carried out difference analysis,correlation and regression analysis to analyze the difference of risk factors and relative power on qEEG data between the two groups.Result1.The proportion of diabetes mellitus,Low-density lipoprotein-cholesterol(LDL-C)and total cholesterol(TC)in the cognitive impairment group after stroke was higher than that in the control group(P<0.05).There was no significant difference in age,gender,education,blood pressure,coronary heart disease,High-density lipoprotein-cholesterol(HDL-C),triacylglycerol(TG)and Body Mass Index(BMI)between the two groups(P>0.05).2.The DTABR[(δ+θ)/(α+β)]and DAR(δ/α)values of frontal and parietal lobes in cognitive impairment group were higher than those in control group,the difference was statistically significant(P<0.05);The DTABR and DAR values of central area,occipital lobe and temporal lobe in the cognitive impairment group were also higher than those in the control group,and the difference was not statistically significant(P>0.05).3.The DTABR and DAR values of the frontal lobe,central area,parietal lobe,occipital lobe and temporal lobe in the group with post-stroke cognitive impairment showed no significant difference between the left and right sides(P>0.05).4.The total score of MMSE,space-time orientation,recall,attention and calculation,expression ability and drawing ability in the post-stroke cognitive impairment group were lower than those in the control group,and the difference was statistically significant(P<0.05);The score of immediate memory was lower than that of the control group,and the difference was not statistically significant(P>0.05).5.There was a significant negative correlation between DTABR value and the scores of three sub-cognitive domains of attention and calculation,expression ability and drawing ability(P<0.05 and r<0),but there was no significant correlation between DTABR value and three sub-cognitive domains of space-time orientation,immediate memory and recall(P>0.05);DAR value had significant negative correlation with the scores of four sub-cognitive domains:space-time orientation,attention and calculation,expression ability and drawing ability(P<0.05 and R<0),but had no significant correlation with the two sub-cognitive domains of immediate memory and recall(P>0.05).6.Binary logistic regression analysis showed that DTABR value,DAR value and LDL-C level were independent risk factors of cognitive impairment after stroke.Conclusion1.History of diabetes mellitus,LDL-C and TC levels are risk factors for cognitive impairment after stroke.LDL-C level,DTABR value and DAR value are positively correlated with post-stroke cognitive impairment,and are independent risk factors of post-stroke cognitive impairment.Both DTABR value and DAR value can be used as indicators to evaluate post-stroke cognitive status and predict prognosis,and DAR value is more sensitive.2.The activity relative power of frontal and parietal lobes slow down after acute ischemic stroke.There was no significant difference in the relative power of left and right hemispheres in post-stroke cognitive impairment.3.Cognitive impairment after acute ischemic stroke is mainly manifested in the defects of five sub-cognitive domains,such as overall cognitive status,space-time orientation,recall,attention and calculation,expression ability and drawing ability.
