BackgroundHelicobacter pylori(Hp)is one of the most widely infected bacteria in the world.Infection can lead to chronic gastritis and increases the risk of mucosal and lymphoid tissue lymphomapeptic ulcers and gastric adenocarcinoma.In the early 1980s,Marshall and Warren made the groundbreaking discovery of helicobacter pylori,which began intense research as a pathogen.At present,a large number of studies at home and abroad have shown that eradication of Hp can bring unequal benefits to different populations.Current due to the different parts have different degrees of antibiotics abuse problems of domestic clarithromycin and metronidazole,amoxicillin and fluoroquinolone drugs resistant rate is on the rise,but still have certain regional differences,so this study is to explore the eradication of an ideal scheme of jinan area,in turn,guide the clinical treatment,improve eradication rate and patient satisfaction.ObjectiveTo explore a relatively ideal eradication program in Jinan area,and then guide clinical treatment,improve the eradication rate and patient satisfaction.Research methodsFrom January 2020 to December 2020,235 cases of helicobacter pylori eradication patients in the outpatient department of the Second Hospital of Shandong University were selected as the research object,and they were divided into three groups A,B and C according to different eradication schemes.In group A,amoxicillin 1g bid,clarithromycin 0.5g bid.colloidal bismuth capsule 220mg bid,eprazole 5mg bid;Group B included clarithromycin 0.5g bid,levofloxacin 0.5g qd,pantoprazole 40mg bid and colloidal bismuth capsule 220mg bid.Group C consisted of ornidazole 0.5g bid,clarithromycin 0.5g bid,esomeprazole 20mg bid and colloidal bismuth capsule 220mg bid.The treatment course of the three groups was 14 days,and 1 3C-UBT was reviewed at least one month after the treatment course and telephone follow-up was conducted to compare Hp eradication rate,improvement of digestive tract symptoms and incidence of adverse drug reactions in different groups.ResultsA total of 235 patients who underwent Hp eradication treatment from January 2020 to December 2020 were included in the study,including 227 patients who completed all treatment regimings.The H.pylori eradication rate,clinical symptom improvement rate and incidence of adverse reactions were analyzed by intention-to-treat(ITT)and protocol-to-treat(PP).H.pylori eradication rate:The ITT eradication rate for all patients was 74.9%(176/235).The eradication rate of ITT in group A was 85.2%(75/88),that in group B was 70.3%(38/54),and that in group C was 67.7%(63/93).There was statistical significance(P AB=0.033;PAC=0.006),there was no difference between group B and group C(PBC=0.11).The PP eradication rate of 227 patients was 77.5%(176/227).The eradication rate of PP was 86.2%(75/87)in group A,The eradication rate of PP in group B was 71.7%(38/53);the eradication rate of PP in group C was 72.4%(63/87).71.7%,There is a significant difference between the groups A and B and C(PAB=0.035;PAC=0.025),there was no difference between group B and group C(PBC=0.927).ITT analysis was consistent with PP analysis.Clinical symptom improvement:according to ITT analysis,the total effective rate was 85.2%(75/88)in group A,68.5%(37/54)in group B,and 65.6%(61/93)in group C.The difference between group A and group B and C was statistically significant(PAB=0.018;PAC=0.002),there was no significant difference between group B and GROUP C(PBC=0.717).According to PP analysis,the total effective rate was 86.2%(75/87)in group A,69.8%(37/53)in group B and 70.1%(61/87)in group C.The difference between group A and group B and C was statistically significant(PAB=0.019;PAC=0.010),there was no significant difference between group B and group C(PBC=0.132).ITT analysis was consistent with PP analysis.Incidence of adverse reactions:ITT analysis and PP analysis showed no statistical difference between groups A,B and C(P>0.05).ConclusionsGroup A regimen(amoxicillin 1g bid,clarithromycin 0.5g bid,bismuth capsule of colloid pectin 220mg bid,eprazole 5mg bid)had A high eradication rate of Hp infection and A low incidence of adverse drug reactions in Jinan area,which has A good application and promotion value.
