| Background and purpose:As is widely known,pancreatitis is a serious acute abdominal disease,and recent studies have shown that local and systematic inflammatory responses play critical roles in the pathogenesis of pancreatitis.Pancreatitis brings many bad complications,one of them is called pancreatitis encephalopathy,the disease named pancreatitis encephalopathy is associated with the central nervous system inflammation during pancreatitis.With the development of modern medicine technology,many clinical diseases can be cured,however,until now,clinical therapies of pancreatitis encephalopathy are limited,and current treatments of pancreatitis encephalopathy depend on the treatments of primary disease of pancreatitis.Doctors only can help patients alleviate the symptoms of pancreatitis encephalopathy by easing their pancreatitis.Now doctors and scientists have found that operations of splenectomy can help alleviate the systematic inflammatory responses as well as injury to pancreas,lung and intestinal mucosal barrier,but little is known about its role in the injury of central nervous system during severe acute pancreatitis.Now that resection of spleens by splenectomy can help patients lighten their systemic inflammatory response during acute pancreatitis,we wonder if this kind of operation can help protect patients from pancreatitis encephalopathy.In this background,we hope to investigate the role played by splenectomy in pancreatitis encephalopathy by using rats with severe acute pancreatitis induction and relevant cell experimental study to explore the mechanism involved in it,and we wonder if our study can provide a new thinking for clinical practice.Materials and Methods:The study can be divided into two parts,in the vivo part,we induced severe acute pancreatitis rats by retrograde injection of 3.5%sodium taurocholate into their biliopancreatic duct,while splenectomy were performed at the same time during the induction operations.In the vitro part,we stimulated BV-2 and HT-22 cells with 0.0625%,0.125%,0.25%,0.5%trypsin.After that,Pancreatic or brain tissue and serum samples were collected and subjected to histologic analyses.We also analyze body weight changes and survival rates of each group to assess the severity of the severe acute pancreatitis disease.Open field tests,new object recognition tests,tail suspension tests and forced swimming tests were used to assess the injury of behavior and emotion of the rats.Western blotting,ELISA,brain water content and CCK8-kit were used to explore the deep mechanism involved in the concrete effects of splenectomy during the disease.Results:(1)Splenectomy can reduce the severity of severe acute pancreatitis.(2)Splenectomy can alleviate pancreatic encephalopathy.(3)Splenectomy can mitigate brain damage of pancreatic encephalopathy rats.(4)Splenectomy help reduce the activation of NLRP3 inflammasome in pancreas and brain and can ease inflammation response.(5)Trypsin induce the activation of NLRP3 inflammasome in microglia cells and increase the release of pro-inflammatory cytokines and decrease survival rates of hippocampal neurons.Conclusion:Splenectomy can reduce the expression and activation of NLRP3 inflammasome,thus alleviate central nervous system inflammation during severe acute pancreatitis and mitigate the severity of pancreatitis encephalopathy. |
