Analysis Of Risk Factors Of Adenovirus Pneumonia Complicated With Bronchiolitis Obliterans In Children

Objective:Adenovirus has high mortality and can leave serious sequelae.This paper mainly discusses the related risk factors of adenovirus pneumonia complicated with bronchiolitis obliterans(BO)in children,so as to provide basis for clinical diagnosis and treatment and accurate judgment of bronchiolitis obliterans,and reduce the occurrence of serious sequelae.Method:This paper collected the clinical data of 316 children with adenovirus pneumonia admitted to the First Affiliated Hospital of Xiamen University from January 1,2019 to June 30,2020,and evaluated whether there was bronchiolitis obliterans according to the follow-up clinical manifestations,imaging and complications The patients were divided into BO group and Non BO group.The clinical data,laboratory examination and imaging data were retrospectively analyzed.Results:1.316 cases of adenovirus pneumonia in children with a total of 69 cases of BO group.The length of hospital stay,duration of oxygen use,duration of mechanical ventilation,AST level,heat peak,heat course,LDH,PCT,timing of hormone use and mechanical ventilation in BO group were greater than those in Non BO group.The levels of age,albumin,D-dimer,fibrinogen and CD3+in BO group were lower than those in Non BO group,and the difference was statistically significant(P<0.05).2.Multivariate logistic regression analysis showed that there were significant differences between the two groups in age,hormone use after 7 days of onset and the following indicators(P<0.05).ROC curve showed that the sensitivity and specificity of distinguishing BO from Non BO were higher when hospital stay>8.5 days,AST>72.5u/l,duration of Mechanical ventilation>5.5 days and LDH>604.5 IU/L were the cut-off values.Conclusion:In this study,we found that the younger the age,the higher the possibility of BO after adenovirus infection.If the length of hospital stay is more than 8.5 days,the more advanced oxygen supply mode or oxygen dependence time is needed,especially the duration of mechanical ventilation is more than 5.5 days,the levels of AST and LDH are increased(604.5 IU/L)and the use of hormone 7 days after the course of the disease all indicate that BO may occur in the follow-up,so it is necessary to be vigilant and treated as soon as possible...
