Objective:To study the relationship between CT findings and the expression level of microvessel density,EGFR,GPC-3,Ki-67 in cancer tissues,and to investigate the clinical value of CT in evaluating microangiogenesis of hepatocellular carcinoma.Methods:Forty six patients who had hepatocelluar carcinoma with complete clinical data and imaging data were gathered from December 2014 to December 2019 in our hospital.The diagnosis of all patients was confirmed with surgery.All cases were performed routine CT scans and three-phase contrast enhancement before operations.The expression levels of microvessel density(MVD),epidermal growth factor receptor(EGFR),glypican-3(GPC-3)and cellular proliferative nuclear antigen(Ki-67)in cancer tissues were detected by immunohistochemistry,then combined with pathological grading results and CT signs for comparative analyses.The correlation between the expression of EGFR,GPC-3,Ki-67 and histopathological grade in cancer tissues was analyzed;to explore the relationship between the expression levels of EGFR,GPC-3,Ki-67 and the different CT imaging features(size,margin,tumor necrosis,metastasis,enhancement patterns)of liver cancer;to evaluate the relationship between MVD,EGFR,GPC-3 and Ki-67.Results:1.The relation between CT appearance,pathological grade and MVD:MVD was highly expressed in the groups with larger tumor size(>3cm),and tumor necrosis(P=0.007;P<0.001).Tumor margin was not correlated with MVD(P=0.219).There was no obvious relationship at MVD level between metastasis and non-metastasis(P=0.082).In HCC with arterial blood and double blood supply,MVD expression level was higher than that of poor blood supplies(P<0.05).At the same time,the difference between the arterial blood supply and double blood supply groups was statistically significant(P<0.05).The pathologic grading of HCC was positively correlated with its MVD,in low differentiation group and high differentiation group,in low differentiation group and medium differentiation group(P<0.05).2.The relationship between CT appearance,pathological grade and EGFR:In HCC tissues,the positive expression rate of EGFR was more highly expressed in the groups with larger tumor size(P=0.010),tumor necrosis(P=0.018)and blurred margin(P=0.029).There was no relation between EGFR expression level in tumor metastasis and enhancement patterns(P=0.165;P=0.074).The expression level of EGFR was correlated with the pathological differentiation and the poor pathological differentiation which always have high expression of EGFR in HCC tissues(P=0.008).3.The relationship between CT appearance,pathological grade of hepatocellular carcinoma and GPC-3:In HCC tissues,the positive expression rate of GPC-3 was more highly expressed in the groups with tumor larger size(P=0.016),tumor necrosis(P--0.005)and metastasis(P=0.047).There was no obvious relation between GPC-3 and tumor margin(P=0.179).In HCC tissues,the positive GPC-3 expression rates of poor blood,double blood and arterial blood supply were 20.00%,27.20%,and 80.00%and the GPC-3 of the three groups gradually increased(P--0.001).The positive rate of GPC-3 was significantly related to the pathological grade(P=0.001).4.The relationship between CT findings,pathological grade and Ki-67:In HCC tissues,the positive expression rate of Ki-67 was more highly expressed in the groups with tumor larger size(P=0.032),and tumor necrosis(P=0.002).There were no statistically significant difference between the tumor margin,metastasis,enhancement patterns and Ki-67 expression(P=0.090;P=0.262;P=0.152).The positive rate of Ki-67 was significantly related to the pathological grade(P=0.015).5.The relationship between EGFR,GPC-3,Ki-67 and MVD in liver cancer:In HCC tissues,the MVD values of the positive and negative EGFR expression groups were 66.30±23.04 and 49.64±19.15.The MVD values of the positive and negative GPC-3 expression groups were 70.76±21.12 and 46.39±17.76.The MVD values of the positive and negative Ki-67 expression groups were 68.74±19.32 and 48.42±23.77.The differences were statistically significant(P=0.023;P<0.001;P=0.003).6.The relationship between EGFR,GPC-3 and Ki-67 in HCC tissues:The expression of EGFR and GPC-3 were consistent(Kappa=0.4309,P=0.002).The expression of EGFR and Ki-67 were not consistent(P=0.061).The expression of GPC-3 and Ki-67 were consistent(Kappa=0.309,P=0.036).Conclusion:1.The CT features of hepatocellular carcinoma are closely related to the expression of EGFR,GPC-3,Ki-67 and MVD.CT signs can reflect the biological characteristics of hepatocellular carcinoma2.The expressions of EGFR,GPC-3,Ki-67 and MVD can indicate the pathological grade of HCC.3.EGFR,GPC-3 and Ki-67 can promote tumor microangiogenesis.4.The expressions of EGFR and Ki-67 are consistent with GPC-3,which can promote the occurrence and development of HCC.
