A Polysaccharide-based Injectable Hydrogel For Hemostasis And Postoperative Anti-adhesion After Hepatectomy

Abstract/Summary:
Hemostasis and prevention of postoperative adhesions after hepatectomy are urgent problems to be solved clinically.To date,most of clinical hemostatic agents only focus on hemostasis and lack the function of preventing postoperative tissue adhesion.Therefore,developing hemostatic agent both capable of hemostasis and anti-adhesion for using after hepatectomy is challenging.In this work,we firstly synthesized dodecyl-modified N-carboxyethyl chitosan(DCEC)and oxidized konjac glucomannan(OKGM),and the successful synthesis of two polysaccharide derivatives was confirmed by the nuclear magnetic resonance spectrum.Then,an injectable hydrogel was prepared by the Schiff base reaction.The main components of the hydrogel are natural polysaccharides,contributing good cytocompatibility,hemocompatibility and degradability to hydrogel.Hydrogel based on dynamic Schiff base bond exhibited self-healing ability and tissue adhesion properties,whose adhesion strength on pig skin could reach 20 k Pa.The introduction of dodecyl groups in the DCEC endowed the hydrogel with excellent antibacterial capability(the inhibition rate of S.aureus and E.coli reach more than 90%)and in vitro coagulation performance.The notable hemostatic efficacy of the hydrogel was confirmed in a rat partial hepatectomy model,which could achieve hemostasis at about 44±7 s,and the quantity of blood loss was about 426±125 mg,which was superior to commercial gauze(134±18 s of hemostasis time,713±79 mg of blood loss).Moreover,the hydrogel also could prevent postoperative adhesions in situ after hepatectomy due to the combination of barrier function and down-regulating the inflammatory factor TNF-αand the pro-fibrotic factor TGF-β1.Thus,DCEC/OKGM injectable hydrogel is potentially valuable for preventing the bleeding and postoperative adhesions in hepatectomy.
