The population statistics of the National Bureau of Statistics in 2019 show that the total number of elderly people in China is increasing, and our country is accelerating to enter the aging society, facing the problems of rapid growth and large total. At the same time, the health problems of the elderly are also gradually highlighted, showing that the increasing life expectancy of human beings is accompanied by the aging of illness, and the low health literacy of the elderly leads to the decline of their quality of life and so on.In view of this phenomenon, the elderly education aimed at improving the health literacy of the elderly has gradually developed, and has become an important means to improve the quality of life of the elderly and to realize healthy aging and active aging. But the current service has a single curriculum content, mode administration and supply and demand do not correspond to the situation, which leads to many elderly people to improve their health literacy needs can not be met. With the development of professional social work,it provides a new way of intervention for the health education of the elderly. Therefore, this study mainly through the group work professional method and the pattern, enhances the group member's health accomplishment, satisfies its health need, thus provides the effective experience for the exploration old age health literacy enhancement. In this study, the health self-assessment and health literacy survey of 46 elderly people in the landscape district of nanjing city were conducted by the survey of health literacy of chinese citizens. The statistical analysis of the data showed that the health literacy of the elderly in the district was low.On this basis, the study selected 12 elderly people ranked in the bottom third of the health literacy score to form a "Kang Le" group, aimed at improving his health literacy through professional social work services. The researchers adopted different professional methods and techniques at different stages of the group. In the group preparation period, the researchers conducted a needs assessment of the health literacy of the elderly, and the results showed that the group members were deficient in three aspects of health knowledge and concept:" healthy correct understanding "and" aging correct attitude "and" chronic disease related knowledge ".In terms of health-related basic skills, the elderly lack"self-care skills" and "ability to obtain health information ";the researchers designed ten group activities based on the health literacy improvement needs of group members. In the group start period, group formation period, group conflict period and group maintenance period, based on group motivation theory and self-efficacy theory, using the reciprocal model and development model of group work, the group activity content and group activities are designed for three aspects of health literacy, and the health literacy survey of group members is conducted again at the end of the group to test the effect of group activities. The researchers found that after the intervention of group work on the health literacy of the elderly, the scores of group members' health literacy generally improved, the goal of group activities was basically completed, and ten group activities achieved better results,so group work to improve the health literacy of the elderly has applicability and professionalism;but after reflection, the researchers believe that the improvement of health literacy and the realization of healthy aging require multiple and comprehensive intervention and service.
