Object:To explore the therapeutic effect of 577 nm micropulse laser combined with Ranibizumab on Diabetic Macular Edema(DME).Methods:56 cases(56 eyes)of DME patients diagnosed in the Ophthalmology of hongqi hospital affiliated to Mudanjiang Medical College from January 2018 to December2018 were included.According to different treatment schemes,divided into two groups,28 eyes in each group.Experimental Group: 577 nm micropulse laser combined with Ranibizumab group(3 + PRN);Control group: single Ranibizumab group(3 + PRN).In the Experimental Group,577 nm laser was used in the macular area for 2 weeks after the first injection of Ranibizumab,and the interval of repeated photocoagulation was at least 3months.Recording the BCVA? IOP?CMT?FFA? the number of injections in before and1 month,3 months,6 months and 12 months after treatment.The difference of curative effect between the two groups was compared.And observe whether there are endophthalmitis,cardiovascular and cerebrovascular adverse reactions.Results: Experimental Group and Control group before and after treatment,the BCVA and CMT in the Experimental Group were significantly improved(Ftime = 33.93,P < 0.05)(F time = 146.730,P < 0.05);In BCVA,there was no significant difference in the interaction effect between the time point and the group(F time * group = 0.466,P > 0.05);There was no difference in BCVA improvement between the two treatment methods(F group = 0.681,P > 0.05).In CMT,there was significant difference between time point and group(F time * group = 8.112,P < 0.05);and(F group = 0.681,P = 0.413)indicated that there was statistical difference between the two groups.Both groups had significant differences in CMT at different time points after treatment,(t = 0.100,2.050,2.648,3.452,2.847,P < 0.05);12 months after treatment,the average annual injection times of Ranibizumab in the Experimental Group and Control group were(4.39 ± 1.19)?(5.25 ±1.48),the difference was significant(t = 2.382,P < 0.05).FFA results showed that the effective rate of the Experimental Group was higher than that of the control group(P <0.05).There were no serious adverse reactions.Conclusions: 1.The visual acuity of the patients with DME can be improved by the two treatments,But both curative treatment are no significant difference;2.The two treatment schemes can reduce the macular edema of DME patients,but Experimental Group is obvious;3.577 nm micropulse laser combined with Ranibizumab can reduce the number of injection needles of Ranibizumab;4.Two groups can reduce vascular leakage,and the Experimental Group has better effect.
