Objective : To compare the clinical efficacy of unilateral biportal endoscopic technique(UBE)and large channel endoscopy(Lusta)for the treatment of degenerative lumbar spinal stenosis, and to summarize the technical notes,to clarify the safety and effectiveness, and to provide evidence support for clinical application of two techniques. Methods Retrospectively analyzed 48 patients with degenerative lumbar spinal stenosis treated between November 2020 and July 2020 in the department 1of Neck, shoulder and lumbar leg pain in Sichuan Orthopaedic Hospital.According to different surgical methods,The patients were divided into UBE group(n=26)and large channel endoscopy(Lusta)group(n=22)as A and B,the preoperative clinical data, operation time, postoperative hemoglobin drop, length of hospital stay, and surgical complications between the two groups. The visual analogue score(VAS),The Oswestry disability index(ODI),in the last follow-up.the modified Mac Nab standard was used to evaluate the excellent and good rate, the key points of the technique were summarized. Results Both groups successfully completed the operation.UBE group was significantly better than large channel endoscopy(Lusta)group in operation time(P<0.05),and the decrease of postoperative hemoglobin in the UBE group was significantly greater than in the large channel endoscopy(Lusta)group in operation time(P<0.05),and there was no statistical significant difference in the length of hospital stay and post operative complications between the two groups(P>0.05),the complication rate was 8% in the UBE endoscopy group and 9% in the large channel endoscopy(Lusta)group. The VAS score and ODI score of the two groups were all significantly improved after treatment(P < 0.05),but there was no significant difference in VAS score and ODI score between two groups(P > 0.05).Conclusion Unilateral biportal endoscopic technique(UBE)and large channel endoscopy(Lusta)both achieve good clinical efficacy in the treatment of degenerative lumbar spinal stenosis. Both are effective minimally invasive surgical methods for the treatment of degenerative lumbar spinal stenosis, but UBE has shorter operation time, higher decompression efficiency and increased blood loss.
