Experimental Study On The Effects Of Different Concentrations Of Hypertonic Glucose On The Expression Of Collagen Type Ⅰ, Ⅲ And IL-1β In A Rabbit Model Of Achilles Tendinopathy

Objective: To establish a rabbit animal model of Achilles tendon disease and observe the effects of different concentrations of hypertonic glucose on the histological morphology of Achilles tendon,the expression of type Ⅰ and Ⅲ collagen and IL-1β,and to explore the mechanism and dose-effect relationship of hypertonic glucose in the treatment of tendinopathy,so as to provide theoretical basis for its clinical application.Forty New Zealand white rabbits were randomly divided into group K(blank control group),group S(normal saline intervention group),group A(12.5%GS intervention group),group B(25%GS intervention group)and group C(50%GS intervention group)after 1 week of adaptive feeding,with A total of 5groups,8 rabbits in each group.A,B,C and S group were induced by PGE2 to establish rabbit animal model of Achilles tendon disease.After the successful establishment of the animal model,group A,group B,group C and group S received right Achilles tendon injection with 12.5%GS,25%GS,50%GS and 0.9%normal saline,once A week,A total of 4 times.Group K was not given any intervention treatment.One week after the intervention,the animals were killed by air embolization.The experimental indexes were observed successively and the differences between the groups were compared.(1)The morphological structure of tendon tissue was observed under light microscope;(2)The changes of tendon fiber density and diameter were observed by transmission electron microscopy.(3)The mRNA expression levels of type Ⅰ and Ⅲ collagen and IL-1βin Achilles tendon tissues were detected by q RT-PCR.Results:The animals in this group were in good condition during the experiment,and no adverse events and accidental death occurred.(1)The general observation results of Achilles tendon:color composition of A,B and C was uneven,slightly shiny,and the surrounding tissues were slightly edema,no adhesion,obvious vascularization,and some adipose tissue appeared;The color of group S was light yellow,dull,and soft,with edema and adhesions in the surrounding tissues,and some fatty tissues appeared.Group K was white in color,shiny and tough in texture,with no edema,adhesion or obvious vascularization in the surrounding tissues.(2)The results of Masson staining showed that the fibers of the Achilles tendon in group A were slightly disordered,slightly interrupted and wavy.In group B,the fibers of Achilles tendon were moderately disordered,moderately fractured and uneven in color.Group C had slightly disordered,orderly and uniform arrangement of tendon fibers.The fibers of the Achilles tendon in group S were disordered,obviously broken and wavy.The fibrous tissues of the Achilles tendon in group K were orderly,continuous and uniform in color.(3)The results of fluoroscopic electron microscopy were as follows:the collagen fibers in the Achilles tendon tissue of the intervention groups with different concentrations of GS were coarser than those of the S and K groups,and the collagen fibers in the Achilles tendon of the A and C groups were coarser than those of the B group.Compared with group B,groups A and C had lower fiber density,larger fiber cross section and longer fiber diameter(P<0.001).(4)mRNA expression of type I collagen:compared with group K,the expression level of group S was significantly decreased(P<0.05);Compared with group S,the expression level of groups A and C was significantly increased(P<0.05);Compared with group B,the gene expression level in groups A and C was significantly increased(P<0.05).There was no significant difference in the expression level between group A and group C(P>0.05).(5)mRNA expression of type III collagen:compared with group K,the level of group S was significantly increased(P<0.05);Compared with group S,gene levels in groups A,B and 5C were significantly decreased(P<0.05);Compared with group B,the gene expression level in groups A and C was significantly decreased(P<0.05).There was no significant difference in the expression level between group A and group C(P>0.05).(6)IL-1βexpression:Compared with group K,the expression level of group S was significantly increased(P<0.05);Compared with group S,the expression levels of groups A,B and C were significantly decreased(P<0.05);Compared with group B,the expression level of group A and C was significantly decreased(P<0.05).There was no significant difference in the expression level between group A and group C(P>0.05).Conclusion:(1)the hypertonic glucose can promote animal tendons disease modelⅠ,Ⅲcollagen synthesis,change the state of tendon fiber density,diameter,and arrangement.(2)Hypertonic glucose can reduce or inhibit the inflammatory response by down-regulating the expression level of IL-1β,and provide conditions for tendon tissue repair.(3)Hypertonic glucose can promote the repair of tendonopathy closely related to its concentration.12.5%GS and 50%GS can better promote the repair of type Ⅰ and Ⅲ collagen,has a better effect on reducing or inhibiting inflammatory response.
