Background: At present,the highest recognition for the diagnosis of prostate cancer is prostate biopsy,but the positive rate of puncture will be affected by serum PSA(prostate specific antigen).BMI(body mass index),PV(prostate volume,Prostate volume(prostate volume)and age,and there have other factors can affect the PSA,leading to the decrease of the accuracy of prostate puncture,especially for patients with PSA < 10 ng / ml,which makes some patients need a second or even multiple prostate puncture to make a clear diagnosis,which is not conducive to the diagnosis of prostate cancer,but also causes great trauma to the patient's body.Some foreign scholars put forward the concepts of PSAM(total mass of prostate specific antigen)and PSAMR(total mass ratio of prostate specific antigen),hoping to improve the diagnostic efficiency by correcting serum PSA.Objective: To study the relationship between PSAM and PSAMR at different levels of PSA and BMI and prostate puncture positive,and to explore whether PSAM and PSAMR can improve the clinical diagnostic efficacy of PSA in prostate puncture.Methods: from January 2017 to October 2020,571 patients with puncture in Qingdao municipally Hospital were selected,and their clinical data were analyzed retrospectively The correlation between each index and prostate puncture positive was evaluated by multivariate logistic regression analysis with positive prostate puncture results as the dependent variable.Paired t test was used to compare the differences of PSAM,PSAMR and PSA in different groups SPSS software was used to draw the receiver operating characteristic curve,calculate and compare the area under the curve of each factor.Results: With positive puncture as the dependent variable,SPSS software was used for multivariate logistic correlation analysis.The results showed that PSAMR,PSAM,BMI and PSA were risk factors for positive puncture results,The P values of PSAMR PSAM,BMI and PSA were 0.007,0.018,0.039 and 0.032,respectively.The difference was statistically significant(P < 0.05).The or value of age was 1.048,P = 0.144.The positive rate of puncture was 26.04% in PSA 4-10 ng / ml group and 58.30% in PSA 10-100 ng / ml group,The values of PSAMR,PSAM and PSA in patients with positive prostate puncture were significantly higher than those in patients with negative prostate puncture.Paired t-test was performed between different puncture result groups,and the difference was statistically significant(P < 0.05).The data of serum PSA 4-10 ng / ml group and PSA 10-100 ng / ml group were drawn to draw the subject characteristic curve,PSAMR,PSAM and PSA The area under the receiver characteristic curve of PSA was greater than 0.5,and the area under the receiver characteristic curve was PSAMR > PSAM > PSA;the positive puncture rate was 52.62% in the BMI group of18.5-24 kg / m2,and there were significant differences in PSAMR,PSAM and PSA among different puncture results,and the difference was statistically significant(P <0.05): the positive puncture rate was 36.62% in the BMI group of > 24 kg / m2,The paired t test of PSAMR,PSAM and PSA among different puncture results showed that there was significant difference in PSAM and PSAMR among patients with different puncture nodes(P < 0.05),but there was no significant difference in PSA among patients with different puncture results(P > 0.05).Conclusion: 1.PSAM and PSAMR were independent risk factors for positive puncture results.Prostate volume was a protective factor for positive puncture results.Age had no effect on prostate puncture results 2.The predictive ability of PSAMR and PSAM for positive puncture results is higher than that of PSA,especially in the gray range of PSA(4-10 ng / ml);3.The positive predictive ability of PSAM and PSAMR for overweight patients was higher than that of PSA;4 PSAM and PSAMR are more effective than PSA in the diagnosis of positive prostate puncture.
