| ObjectiveSummarized the clinical experience of Professor Yao Naili in treating chronic gastritis(CG)complicated with depression to inherit and promote the academic views of illustrious senior traditional Chinese medicine,and evaluated the efficacy and safety of professor’s empirical prescription Jianpi Tiaogan Jieyu Decoction.MethodsThrough collecting literature and clinical cases to systematically summarized the clinical experience of Professor Yao Naili in diagnosis and treatment of CG complicated with depression from diagnosis and treatment of the diseases.And through prospective case series studies to evaluate Jianpi Tiaogan Jieyu Decoction.68 patients with CG complicated with depression due to stagnation of liver qi and spleen deficiency in gastroenterology of Guang’anmen Hospital,China Academy of Chinese Medical Sciences from September 2020 to December 2020 were selected.All the patients were treated by 400 ml Jianpi Tiaogan Jieyu Decoction each morning and evening for 4 weeks,and then be followed for 4 weeks.The scores of the CG TCM symptom scale,the 9-item Patient Health Questionnaire(PHQ-9),and the 17-item Hamilton Depression Scale(HAMD-17)score were recorded before medication and 4 weeks after medication and follow-up.The safety indexes were recorded before medication and 4 weeks after medication.The differences in curative effect before treatment,after treatment and during follow-up were analyzed by SPSS 25.0.Results1.Summary of clinical experience:Professor Yao Naili points that,the disease is caused by the six evils of the exogenous,eating disorders,emotional insufficiency,and deficiency of spleen and stomach,and based on the injury of spleen and stomach.The basic pathogenesis of the disease is liver dysfunction and dysfunction of the spleen in transport,and deficiency of spleen and stomach plays a decisive role.Therefore,Professor Yao Naili puts spleen supplementation and transportation as the first priority in treatment,and also relieves the liver qi,treats the disease by Jianpi Tiaogan Jieyu Decoction which invigorates the spleen,regulates the liver and relieves depression.2.Clinical efficacy:68 patients were included in the study,and 65 clinical observations were completed due to loss to follow-up,failure to adhere to treatment,etc.(1)Evaluation of curative effect of TCM symptoms:According to reduction rate of TCM symptoms of CG,6 cases were clinically cured(9.2%),31 cases were markedly effective(47.7%),24 cases were effective(36.9%),and 4 cases were ineffective(6.2%)after 4 weeks of treatment,and the total effective cases was 61(93.85%).(2)Evaluation of the efficacy of depression:According to the PHQ-9 reduction rate,17 cases were clinically controlled(26.2%),34 cases were markedly effective(52.3%),10 cases were effective(15.4%),and invalid 4 cases(6.2%)after 4 weeks of treatment,and the total effective cases was 61(93.85%).According to the reduction rate of HAMD-17,14 cases were clinically controlled(21.5%),37 cases were markedly effective(56.9%),11 cases were effective(16.9%),and 3 cases were invalid(4.6%),the total effective cases was 62(95.38%).(3)Evaluation of recurrence:According to the TCM symptom score of CG,the number of recurrence cases was 14(21.5%).Based on the PHQ-9 score,the number of recurrence cases was 13(20.0%).Based on the HAMD-17 score,the number of recurrence cases was 14(21.5%).The three are roughly equivalent.(4)Safety evaluation:During this clinical study,none adverse reaction occurred.ConclusionProfessor Yao Naili has unique insights into CG complicated with depression based on years of clinical experience,and the treatment method has the advantages of traditional Chinese medicine.Jianpi Tiaogan Jieyu Decoction can effectively improve the clinical symptoms and depression of patients with definite curative effect and good safety. |
