Objective:To study the clinical effect of treating acute exacerbation of chronic obstructive pulmonary disease(COPD)with intradermal acupuncture and regulating the liver and regulating the lung.To further explore the clinical effect of intradermal acupuncture for regulating liver and regulating lung combined with medicine in the treatment of acute exacerbation of chronic obstructive pulmonary disease..Methods:In this study,60 patients with acute exacerbation of chronic obstructive pulmonary disease(phlegm-heat obstructing lung and stagnation of liver-qi syndrome)were randomly divided into test group and Control Group in Tongzhou District of Beijing University of Chinese Medicine Dongzhimen Hospital,the Control Group was treated with Western medicine(2-3 L/min of oxygen inhalation,2-3 1/min of piperacillin sodium and Sulbactam sodium with antibiotics,2-3 1/min of ambroxol injection,30 mg of ambroxol injection and 30 MG of ambroxol tablet),2.5 ml of compound ipratropium bromide+2 ml of sodium chloride+1 mg of budesonide suspension for 3/day nebulization,0.3 g of doxofylline injection for 1/day,0.1 g of theophylline sustained-release tablets for 2/day),the experimental group was treated with intradermal acupuncture combined with Western Medicine(selected points:Ganshu,Feishu,Taichong,Taiyuan).Operation Method:The site of selected acupoints is determined.After the body surface of the given acupoints is disinfected by alcohol,the needle body of the intradermal needle is aligned to the acupoints,and the needle is inserted vertically into the acupoints,the needle handle part originally attached to the disinfectant adhesive tape can stay flat and firmly fixed on the local skin.Needle retention time:24-48 hours.The first day of admission was the first time for acupuncture,and the interval between two acupuncture treatments was 1 day.The clinical symptoms,CAT score,blood routine test,c-reactive protein,anxiety and depression were observed and compared between the two groups during 10 days of treatment.Results:1.Baseline comparison before treatment:There was no significant difference in sex and age between control group and Test Group(P>0.05).2.The two groups were compared:29 cases in the test group,4 cases in clinical control(13.79%),16 cases in apparent effect(55.17%),9 cases in effective effect(31.03%),0 cases in no effect(0%),the total effective rate was 100%.Control Group(29cases),Control Group 3cases(10.34%),effective grouplOcases(34.48%),effective group 14 cases(48.28%),ineffective group 2cases(6.90%),the total effective rate was 93.1%,there was no significant difference between the two groups(P>0.05).3.TCM Syndrome score:There was no significant difference between the two groups before Treatment(P>0.05).After treatment,the scores of both groups were lower than those before treatment,and there was significant difference between the two groups(P<0.05).The Test Group was better than the control group in improving the total score of TCM Syndrome(P<0.05).There was no significant difference between the two groups before Treatment(P>0.05).There was no significant difference between the two groups in the treatment of cough,expectoration,wheezing and red(P>0.05).There was significant difference in the curative effect between the two groups(P<0.05).4.Comparison of anxiety and depression:There was no significant difference between the two groups before Treatment(P>0.05).After treatment,the scores of anxiety and depression in the test group were significantly different(P<0.05),the improvement of anxiety and depression in the test group was better than that in the control group.5.There was no significant difference in WBC,neutrophil percentage and CRP between the two groups before and after Treatment(P>0.05)6.MMRC score and CAT score:There was no significant difference in MMRC score and CAT score between the two groups before Treatment(P>0.05).There was no significant difference in MMRC score and CAT score between the two groups after Treatment(P>0.05).Conclusion:From the analysis of clinical effect,the test group was better than the control group in improving the total score of TCM Syndrome(P<0.05).Among the scores of TCM syndromes,the improvement of symptoms in the experimental group was better than that in the Control Group(P<0.05).The effect of the Test Group on anxiety and depression was better than that of the Control Group(P<0.05).The treatment of intradermal acupuncture for regulating liver and regulating lung combined with Western medicine can improve the symptoms of chronic obstructive acute exacerbation,such as restlessness,irritability,fullness of the chest,dry stool,etc.,and alleviate the state of anxiety and depression,and No side effects,easy operation,for COPD patients with acute exacerbation of anxiety and depression to provide a certain basis for treatment.
