Summary And Analysis Of Professor Chen Tongyun's Experience In Treating Chronic Urticaria

Research significance:Chronic urticaria is one of the common skin diseases in dermatology department.Its clinical characteristics are long course of disease,and easy to relapse.Because of the intolerable itching,it brings certain impact to patient's life,work and psychological health.Western medicine treatment can only relieve the pain of patients for a while,and it is easy to repeat after drug withdrawal.Traditional Chinese medicine treatment often has good curative effect,and the side effect is small.In clinical practice,Professor Tongyun Chen often encounters patients who are difficult to recover from repeated medical treatment.Traditional Chinese medicine treatment can effectively improve the skin lesion status of patients,reduce the number and time of recurrence,and improve the quality of lifeObjective:By collecting and summarizing Professor Tongyun Chen's medical cases in the treatment of chronic urticaria,this paper analyzes its medication and prescription rules,studies Professor Tongyun Chen's experience in the treatment of chronic urticaria,provides relevant data support for inheriting Professor Tongyun Chen's academic thought,and provides clinical guidance for future generations in the treatment of chronic urticariaMethods:This study collects 495 prescriptions for chronic urticaria from Professor Tongyun Chen from January 2010 to December 2020 in the outpatient department of Beijing Traditional Chinese medicine hospital.Through the analysis of data by Traditional Chinese Medicine Inheritance Software and SPSS 20 Software,the frequency distribution,Si Qi Wu Wei,meridians,core drug composition,sex and age distribution of outpatients are obtained by using frequency analysis,cluster analysis,association rules,and usage statistics.Result:A total of 495 prescriptions were collected,involving 156 traditional Chinese medicines,with a total frequency of 7974 times,with an average of 16.11 herbs per prescription,including 21 herbs with high frequency(?100 times).The top 20 times of single herb frequency were Baixianpi 472 times(95.35%),Haitongpi 458 times(92.53%),Wumei 440 times(88.89%),Gancao 434 times(87.68%),Difuzi 406 times(82.02%),Fulingpi 400 times(80.81%),Dongguapi 379 times(76.57%),Mahuang 366 times(73.94%),Chixiaodou 348 times(70.30%),Lianqiao 338 times(68.28%),Fangfeng 330 times(66.67%),Danggui 317 times(64.04%),Huangqi 309 times(62.42%),Baizhu 303 times(61.21%),Digupi 267 times(53.94%),Jingjie 263 times(53.13%),Wuweizi186 times(37.58%),Kuxingren 178 times(35.96%),Yinchaihu 175 times(35.35%),Dafupi 164 times(33.13%).The top five herb efficacy frequency were water-enhancing and humidity-permeating herbs for 1648 times(26.50%),heat-clearing herbs for 1256 times(20.19%),exterior-relieving herbs for 1063 times(17.09%),deficiency-restoring drugs for 668 times(10.74%),and deficiency tonifying drugs for 563 times(9.05%).The drug frequency was 2104 times for Han(33.94%),429 times for Liang(6.92%),1416 times for Ping(22.84%),2229 times for Wen(35.96%),21 times for Re(0.33%);the drug frequency of five flavors was Xin 1974 times(22.69%),Ku 2678 times(30.78%),Gan 2953 times(33.94%),salty 41 times(0.47%),Suan 1055 times(12.13%);the top three drugs were spleen meridian 3160 times(39.63%),lung meridian 3079 times 61%),liver meridian 2202 times(13.91%).According to the analysis,the core prescriptions were as follows:Baixianpi,Haitongpi,Difuzi,Shenggancao,Wumei,Mahuang;Baixianpi,Haitongpi,Shenggancao,Wumei,Difuzi;Baixianpi,Haitongpi,Wumei,Shenggancao,Fulingpi,Dongguapi;Baixianpi,Shenggancao,Wumei,Difuzi,Fangfeng.From January 2010 to December 2015,a total of 257 prescriptions were isolated,involving 116 herbs,with a total frequency of 4084 times,with an average of 15.89 drugs per prescription.The top 16 times of single herb frequency were Baixianpi 245 times(95.33%),Haitongpi 237 times(92.22%),Gancao 232 times(90.27%),Wumei 227 times(88.33%),Difuzi 197 times(76.65%),Fangfeng 197 times(76.65%),Fulingpi 196 times(76.26%),Danggui 179 times 69.65%),Jingjie 178 times(69.26%),Huangqi 178 times(69.26%),Dongguapi 176 times(68.48%),Baizhu 170 times(66.15%),Mahuang 149 times(57.98%),LIanqiao 138 times(53.70%),Chixiaodou 130 times(50.58%),Digupi 118 times(45.91%).From January 2019 to December 2020 a total of 61 prescriptions were isolated,involving 71 herbs,with a total frequency of 972 times,with an average of 15.93 herbs per prescription.The top 16 times of single herb frequency were Baixianpi 59 times(96.72%),Chixiaodou 58 times(95.08%),Mahuang 57 times(93.44%),Difuzi 54 times(88.52%),Wumei 52 times(85.25%),Fulingpi 50 times(81.97%),Gancao 50 times(81.97%),Haitongpi 50 times(81.97%),Lianqiao 45 times(73.77%),Dongguapi 44 times(72.13%),Digupi 39 times(63.93%),Yinchaihu 35 times(57.38%),Fangfeng 34 times(55.73%),Wuweizi 34 times(55.73%),Baizhu 33 times(54.10%),Danggui 30 times(49.18%).Among the 495 prescriptions,there were 181 female patients(67.29%)and 88 male patients(32.71%);24(8.92%)were under 20 years old,43(15.99%)were between 20 and 30 years old,70(26.02%)were between 30 and 40 years old,68(25.28%)were between 40 and 50 years old,28(10.41%)were between 50 and 60 years old,and 36(13.38%)were over 60 years old.Conclusion:Through data mining,it can be clearly seen that Professor Tongyun Chen's medication experience and rules in the treatment of chronic urticaria.Professor Tongyun Chen thinks that chronic urticaria is mostly related to spleen,lung and liver,and the treatment is mainly based on eliminating dampness and diuresis,clearing away heat and relieving exterior,and at the same time,she also gives attention to astringency and restoring deficiency.The prescription mainly comes from Duopi decoction,Mahuang Lianqiao Chixiaodou Decoction,Guomin Decoction and Danggui Decoction.It also includes Yupingfeng Powder.By comparing the prescriptions of Professor Chen Tongyun in the treatment of chronic urticaria between 2019-2020 and 2010-2015,the changes of the prescriptions can be observed.It is found that the proportion of Yinchaihu and Wuweizi in Professor Chen's prescription has increased,indicating that the use of GuoMin Decoction has been strengthened.
