Objective: The clinical data of patients with liver failure complicated with acute pancreatitis(AP)were retrospectively studied,and the distribution of TCM clinical characteristics and risk factors of AP patients with liver failure were expounded,so as to improve the prevention and treatment ability of these patients and provide theoretical basis for TCM diagnosis and treatment of liver failure complicated with acute pancreatitis.Methods: The clinical data of 269 patients with liver failure admitted to the First Affiliated Hospital of Hunan University of Traditional Chinese Medicine from July2017 to December 2020 were analyzed retrospectively,and a unified information collection form was made to screen out the main symptoms and signs of traditional Chinese medicine.Then,according to whether AP occurred or not,the patients were divided into acute pancreatitis group and non-acute pancreatitis group.The distribution characteristics of main TCM symptoms and signs of the two groups were compared,and the TCM clinical characteristics of patients with liver failure complicated with AP were obtained.Then through chi-square test or t-test single factor analysis,the factors with p < 0.05 were selected for multivariate Logistic regression analysis to explore the risk factors of AP in patients with liver failure.Results:Among 269 patients with liver failure,78(28.99%)had AP,including 73(93.58%)males and 5(6.42%)females;Among 78 patients with liver failure and AP,72 cases(92.30%)were hepatitis B related liver failure,and 6 cases(7.7%)of alcoholic liver failure.Compared with the patients without AP,the patients with AP had more abdominal pain,green tendons in the abdomen,yellow and dark body and eyes,subcutaneous ecchymosis,dark purple tongue,tooth marks on the edge of the tongue and dry stool;Univariate analysis showed that there were significant differences in infection,gallbladder lesions / gallstones,hyponatremia,disease stage,BMI and drinking(P < 0.05);Multivariate logistic regression analysis showed that there were significant differences in gallbladder lesions / gallstones,BMI and drinking(P < 0.05).Conclusion: In AP patients with liver failure,abdominal pain,abdominal exposed tendons,body and eyes color yellow dark,subcutaneous ecchymosis and ecchymosis,tongue purple dark,tongue teeth marks,dry stool symptoms were obvious.Gallbladder lesions / gallstones,BMI and drinking alcohol were risk factors for AP in patients with liver failure.
