Stroke has four characteristics:high morbidity,high recurrence rate,high disability rate,and high fatality rate.Now it has become the leading cause of death in China.Aphasia is one of its common complication and seriously affects the quality of life of patients.Non-fluent aphasia is the main subtype of post-stroke aphasia.Therefore,more attention has been paid to the treatment and rehabilitation of aphasia.Traditional Chinese medicine has a long history of post-stroke aphasia and has a good curative effect.Acupuncture treatment is one of the methods.A large number of studies have shown that on the basis of conventional medical therapy,the " Arousing Brain and Nourishing Marrow " acupuncture scheme combined with language rehabilitation training is effective in treating post-stroke aphasia.However,the mechanism is not yet clear.Among the various brain function detection methods,the electroencephalograph(EEG)has become an important tool for observing the language function of patients with post-stroke aphasia,by virtue of its advantages of non-invasiveness,low cost,easy operation,and high visibility.It provides a data foundation for exploring functional impairments in the loss of aphasia and brain network during recovery.Therefore,in this study," Arousing Brain and Nourishing Marrow " acupuncture therapy combined with linguistic rehabilitation therapy was used as a means of intervention,combined with neurolinguistics scale and electroencephalogram evaluation methods,to explore the effect of acupuncture therapy for poststroke stroke aphasia.Resting state of the brain network changes,so as to more accurately and effectively guide the clinical treatment of post-stroke aphasia.Objective:Using the research method of EEG network,the post-stroke aphasia(PSA)group and health control(Health Control,HC)were analyzed from the characteristics of the scores of the scale and resting state electroencephalogram(EEG)characteristics.The group's resting state EEG network changes,combined with the Chinese Rehabilitation Research Center Aphasia Examination(CRRCAE),the Boston Agnostic Aphasia(BDAE)score,and other neurolinguistic scales Investigate the characteristics of specific EEG signals in patients with post-stroke aphasia,in order to explore the mechanism of brain repair after stroke and the improvement of language function in the EEG network recovery mechanism.Methodology:This study was a prospective paired parallel control study.The PSA group used non-fluent post-stroke aphasia as the study object,and used the "Medication and Awakening" acupuncture program in combination with internal medicine to treat comprehensive treatment programs for 30 days.Resting EEG data collection and neurolinguistic assessment were performed on the same day and at 30 days of treatment.The scales used CRRCAE,BDAE,Mini-Mental State Examination(MMSE),and National Health Institute of Health Stroke Scale(NIHSS),Activities of Daily Living Scale(ADL),The Stroke Aphasic Depression Questionnaire(SADQ),Quality of Life Index Quality of Life Index(QLI).In the HC group,healthy subjects matched with gender,age,and education years were included in the PSA group.The HC group was evaluated by resting electroencephalogram(EEG).At the first time,the PSA group and HC group were subjected to brain Comparison of electromagnetic data,analysis of changes in the brain network of the PSA group compared with the HC group,and correlative analysis with the score of the neurolinguistic scale to explore the injury mechanism of the PSA brain function mechanism.Results:(1)Patients with PSA undergoing 30 days " Arousing Brain and Nourishing Marrow " acupuncture treatment after comprehensive treatment,PSA patients CRRCAE scale sub-items in listening,retelling,speaking,reading,reading,transcription,description,dictation,The calculation ability was improved compared with that before treatment,and the difference was statistically significant(P<0.05).Among them,the scores of listening,retelling,speaking,reading,and transcription were significantly different(P<0.01).The comprehension ability of the CRRCAE scale was There were significant differences between the two types of expression ability before and after treatment(P<0.01).(2)Boston aphasia grade(BDAE),ADL,MMSE,and SADQ scores for PSA patients before and after treatment The National Institutes of Health Stroke Scale(NIHSS)conducted Wilcoxon signed rank test of two related samples.The results showed that after treatment,MMSE,BDAE,and NIHSS were significantly improved compared with before treatment(P<0.01).Before treatment,the difference was statistically significant(P<0.05).(3)Compared with the HC group before treatment,in the PSA group,in the default network,the brain regions with weakened functional connectivity were:right occipital cortex lateral-left sinusoidal gyrus,left lateral frontal gyrus triangle-right zygomatic Middle gyrus,left anterior wedge anterior-left superior apex,left anterior wedge-right fusiform gyrus.In the right frontal network,the brain regions with weakened functional connectivity include:right superior marginal gyrus,right inferior parietal lobe,right frontal lobe,and right frontal middle frontal gyrus.After the treatment in the PSA group compared with before treatment,in the default network,the brain regions with enhanced functional connectivity were:left occipital cortex lateral-right anterior cingulate gyrus,right lateral frontal gyrus trigone-left flank Back and left posterior cingulate gyrus-left middle temporal gyrus,right anterior wedge anterior occipital cortex lateral.In the right frontal network,after the PSA treatment compared with before the PSA treatment,the functional connection strength enhances the right medial forehead to the right inferior parietal leaflet,the right sacral gyrus to the right anteroinferior leaflet,and the right rim The functional connectivity between the gyrus and right inferior lobular brain area is reduced.(4)Pearson correlation analysis of changes in the average functional connectivity values of functionally connective brain regions in the PSA group compared with pretreatment before and after PSA treatment in the right frontal network.There was a certain negative correlation between the strength of the right forehead network connectivity and the expression ability scores of poststroke aphasia patients(r=-0.495,p=0.00.003,Figure 6).This shows that the functional connectivity of the right fronto-cephalic network in patients with post-stroke aphasia is stronger,and the patient's expression ability is worse.In the default network,the Pearson correlation analysis of the average functional connectivity values of the functionally connected reduced brain regions and the linguistic scores scores before PSA treatment showed the strength of functional connectivity in the default network and the ability to express poststroke aphasia patients.There is a positive correlation between the scores(r=-0.172,p=0.018,Figure 7).This may indicate that the lower the functional connectivity strength in the default network of post-stroke aphasia,the better the patient's ability to express.Conclusions:(1)"Arousing Brain and Nourishing Marrow" acupuncture scheme can improve the language function of post-stroke aphasia.(2)The impairment of language function after stroke may becaused by the joint damage of multiple brain function networks.(3)The improvement of the language function of post-stroke aphasiais closely related to the changes of the Default Mode network,Right Fronto Paietal network,which may activate the compensatory function in the brain area and enhance the neuronal electrical activity.
