Objective Comparison of the clinical efficacy of Glissonean Pedicle Transection Method for Hepatic Resection and Pringle Maneuver in primary liver cancer。Methods The clinical data of 50 cases of primary liver cancer performed in the hepatobiliary surgery department of Minda Hospital of Hubei Minzu University from January 2019 to June 2021 were retrospectively analyzed.The way of hepatic vascular inflow was divided into G(Glissonean,n=24)and P(Pringle Maneuver,n=26).To compare the two group from Preoperative(age,sex,presence of cirrhosis,Child-Pugh grade,HGB,WBC,Plt,AST,ALT,ALB,PT),intraoperative(surgical time,intraoperative bleeding,intraoperative blood transfusion rate),postoperative(AST,ALT,A LB,TBIL,PT in 1,3,and 7 days),the situation of abdominal drainage in 7 days after operation,postoperative hospitalization,and complication incidence;Through the statistical description of the above data for groups G and P,to analyze whether the difference between the two data groups was statistical significant or not.Thus,it provides a reference for the selection of hepatic vascular exclusion.Results there is no statistical significance in comparison between general data of the two groups(age,sex,cirrhosis,Child-Pugh,P> 0.05),and also in preoperative clinical indicators(HGB,WBC,Plt,AST,ALT,ALB,TBIL,PT,P> 0.05).ALT and AST are obviously higher on days 1,3 and 7 in group G and P,but group G is significantly lower than P,which has a statistical significance(P <0.05).TBIL in 3 days of G was significantly lower than P(P <0.05),but there is no statistical significance in TBIL in1,7 days,ALB and PT on day 1,3 and 7 between the two groups.The difference between group G and P in surgical time and intraoperative bleeding is statistically significant(P <0.05),and the comparison of intraoperative transfusion rate between the two groups was not significant(P> 0.05).The difference in postoperative complications between the two groups was statistically significant(P <0.05),and group G was lower than group P.The average time in hospital of group G and the 7-day postoperative induced flow are less than these in group P,and the difference was statistically significant(P <0.05)Conclusion 1.There were no serious complications after intermittent Pringle maneuver and Glissonean pedicle transection method,and no perioperative deaths,and both methods had high safety.2.The Glissonean pedicle treats the portal triple section as one,without openning the Glisson sheath,directly breaks the corresponding Glissonean pedicle in the surgical target area,and cut the liver along the ischemic line,which is more in line with the anatomical principle,and the intraoperative bleeding and postoperative peritoneal drainage are less.3.Glissonean pedicle can accelerate postoperative recovery of liver function,which has the advantages of low incidence of postoperative complications and the time in hospital.It is a safe,effective and highly feasible way of hepatic vascular inflow。...
