Objective(s):At present, the pathogenesis of immune thrombocytopenia is not completely clear.Recent studies have shown that genetics, cellular immunity, infection and humoral immunity are all involved in the pathogenesis of ITP.In existing studies, humoral immunity mediated by B cells and cellular immunity mediated by T cells account for a large proportion. The changes of Th1,Th2,Treg and Th17 subsets as well as the proportion of B cells and NK cells in peripheral blood of ITP patients were analyzed by comparing them with healthy people. At the same time, the relationship between Th1,Th2,Treg,Th17 cell subsets and the degree of imbalance of B cells and NK cells and the degree of thrombocytopenia was analyzed, and the relationship between the changes of T cell subsets and the severity of ITP was discussed(the severity of ITP was indicated by the degree of thrombocytopenia). Methods:A total of 35 patients admitted to the Hematology Department of Yuxi People’s Hospital from November 2020 to December 2021 were diagnosed as immune thrombocytopenia, and the proportion of Th1,Th2,Treg and Th17 subsets,CD19+B cells and NK cells in ITP patients and healthy people were detected by flow cytometry.Statistical analysis was performed. Results:The ratio of Th1 cells and Th17 cells in the ITP experimental group was significantly higher than that in the healthy examination group, while the number of Treg cells was significantly lower than that in the healthy examination group. The Th1/Th2 ratio was significantly lower than that in the control group, and the Treg/Th17ratio was lower than that in the control group, with statistical significance(P<0.05).There was a significant linear correlation between the degree of Th1/Th2 cell imbalance and the severity of thrombocytopenia in Treg/Th1 cells. The percentage of CD19+B cells in lymphocytes in peripheral blood of patients with immune thrombocytopenia was significantly higher than the normal value, while NK cells decreased, and the above two differences were statistically significant(P<0.05).Conclusion(s):Patients with immune thrombocytopenia not only have imbalance of Th1 and Th2,but also have imbalance of Treg and Th17 cells. Moreover, the imbalance of Th1/Th2 and Treg/Th17 cell ratios was correlated with the severity of immune thrombocytopenia.CD19+B cells and NK cells in peripheral blood are involved in the pathogenesis of immune platelets.
