| Objective:To investigate the expression of CD117 in high grade neuroendocrine carcinoma of lung.It provides new ideas for clinicopathological diagnosis and treatment.Methods:A total of 60 patients who received radical lung cancer surgery in the First Affiliated Hospital of Anhui Medical University and were pathologically confirmed as lung neuroendocrine tumors were collected from January 2014 to January2021.Among them,14 patients had typical carcinoid,10 patients had atypical carcinoid,20 patients had large cell neuroendocrine carcinoma of lung,and 36 patients had small cell carcinoma of lung.Immunohistochemical En Vision method was used to detect the expression of CD117 in lung neuroendocrine tumor tissues and surrounding tissues.The relationship between CD117 and clinicopathological features of lung neuroendocrine tumor was analyzed by Chi-square test or Fisher’s exact probability test.Kaplan-meier method was used for prognostic analysis.Cox regression model was used for multivariate analysis.Results:1.Among the 80 neuroendocrine tumor tissues,44(55.0%)had CD117 markers≥ 2+.There were 15 large cell neuroendocrine carcinoma(75.0%),29 small cell carcinoma(80.6%),and 0 carcinoid carcinoma(0%).The expression of CD117 in large cell lung neuroendocrine carcinoma and small cell lung carcinoma(G3)were significantly higher than that in typical and atypical lung neuroendocrine carcinoma(G1& G2),and the difference was statistically significant(χ2=37.793,P < 0.0001).2.The high expression of CD117 in lung neuroendocrine tumor tissues was closely related to tumor type,p TNM stage and preoperative chemotherapy,with statistically significant differences(P < 0.05).There was no significant correlation with gender,age,tumor size and smoking status(P > 0.05).3.Overall survival(OS)analysis showed that the CD117 negative group had better prognosis than the positive group,and the difference was statistically significant(P=0.0016).The p TNM stage I/II group had a better prognosis than the p TNM stage III/IV group,and the difference was statistically significant(P < 0.0001).4.Multivariate analysis showed that CD117 expression status and tumor size were independent influencing factors on the overall survival of patients with lung neuroendocrine carcinoma(P < 0.05).Conclusion(1)CD117 is highly expressed in high-grade lung neuroendocrine tumors(LENEC and SCLC),and the high expression of CD117 is significantly correlated with tumor type,p TNM stage,and preoperative chemotherapy.(2)CD117 can be used as a diagnostic marker of lung neuroendocrine tumor.(3)CD117 can be used as a potential prognostic marker for lung neuroendocrine tumors,and its reliability needs to be confirmed by further experiments. |
