Objective In recent years,foreign studies have shown that dapagliflozin not only has a good hypoglycemic effect,but also improves cardiac function and reduces the risk of cardiovascular death.There are few studies on the clinical efficacy and cardiovascular protection of dapagliflozin in Chinese patients with type 2 diabetes and hypertension.This article intends to explore the effect of dapagliflozin on blood glucose,blood pressure,blood lipids,body mass index,carotid artery IMT,cardiac function and left ventricular remodeling in Chinese patients with type 2 diabetes and hypertension,and finds out whether the drug has cardiovascular protective effect on these patients.Methods From October 2020 to July 2021,a total of 125 patients with type 2 diabetes and hypertension who were admitted to the Department of Cardiology and Endocrinology of our hospital and met the inclusion criteria were selected and randomly assigned to the observation group and the control group,with 60 cases in the observation group,On the basis of conventional hypoglycemic drugs,dapagliflozin was added;the control group of 65 cases was treated with conventional hypoglycemic drugs.During the follow-up period,1 patient withdrew from the study due to adverse reactions,1 patient refused to be followed up and dropped out of the study,and the remaining 123 patients completed the follow-up.There were 58 patients in the final observation group,including 35 male patients and 23 female patients.with an average age(68.80 ±11.08)years old,took conventional hypoglycemic drugs as the hypoglycemic regimen.Glycosylated hemoglobin(Hb A1c),fasting blood glucose(FBG),2h postprandial blood glucose(2h PBG),systolic blood pressure,diastolic blood pressure,body mass index(BMI),triglyceride(TG),total cholesterol(TC),high density lipoprotein(HDL),low density lipoprotein(LDL),type B natriuretic peptide(BNP),left ventricular ejection fraction(LVEF),left ventricular end diastolic(LVEDD),interventricular septal diastole thickness(IVSD),and intimal media thickness(IMT)and other data were collected from two groups of patients before and 6 months after treatment.And record the occurrence of adverse reactions such as hypoglycemia,low blood pressure,gastrointestinal reactions and urinary symptoms during the treatment period.SPSS 24.0 software was used for statistical analysis of the data of the two groups of patients before and after treatment.Results After 6 months of treatment,Hb A1 c,FBG,2h PBG,SBP,DBP,BMI,TG,TC,BNP,IVSD and carotid IMT in the observation group were significantly lower than those in the control group(P<0.05),and the LVEF in the observation group was higher than it in the control group(P<0.05).However,there was no statistical significance in HDL,LDL and LVEDD between the two groups(P>0.05).During the follow-up period,there was no significant difference in the incidence of overall adverse reactions between the two groups(P>0.05).Conclusion After 6 months of dapagliflozin treatment,blood glucose,blood pressure and blood lipid levels in patients with type 2 diabetes and hypertension were significantly improved,as well as the cardiac function,left ventricular remodeling and carotid intima-media thickness.At the same time,dapagliflozin has the certain cardiovascular protective effect,and there are no serious adverse reactions occurred during the treatment.
