Objective: To investigate the risk factors of coronary artery calcification(CAC)and the correlation between triglyceride glucose product(Ty G)index and CAC in maintenance hemodialysis(HD)and peritoneal dialysis(PD)patients.Methods: A total of 286 patients who underwent maintenance dialysis treatment in the Nephrology Department of the Second Affiliated Hospital of Anhui Medical University from October 2019 to March 2021 were selected.and Ty G index were calculated.All patients were assessed for the presence of CAC by multislice spiral CT.They were divided into non-calcified group.Statistical analysis of the overall situation and clinical characteristics of CAC in maintenance dialysis patients,compare the differences in clinical data and laboratory parameters in the calcified group without calcification,analyze the correlation between CAC and the above indicators,find the risk factors of CAC by binary logistic regression analysis,and explore the predictive value of risk factors for CAC.Results: 1)The total incidence of CAC among 286 dialysis patients was 157 cases,accounting for 54.9%.2)The age,dialysis age,CRP,Ty G index and i PTH of patients in the calcification group were higher than those in the non-calcification group,and the differences were statistically significant(P < 0.05);however,gender,BMI,hemoglobin,NLR,corrected calcium,serum phosphorus,uric acid,albumin,fasting blood glucose,total cholesterol,triglycerides and other indicators were not statistically different between the two groups(P > 0.05).3)CAC was positively correlated with age(r = 0.453,P < 0.001),dialysis age(r = 0.310,P < 0.001),CRP(r = 0.305,P <0.001),TyG index(r = 0.326,P < 0.001),and i PTH(r = 0.118,P = 0.047)in maintenance dialysis patients by Spearman correlation analysis.4)As shown by binary logistic regression analysis: age(OR = 1.085,95% CI = 1.060 to 1.111,P < 0.001),dialysis age(OR = 1.010,95% CI = 1.004 to 1.017,P < 0.001),CRP(OR = 1.060,95%CI = 1.021 to 1.102,P = 0.003),and Ty G index(OR = 2.309,95% CI = 1.370 to 3.892,P = 0.002)were independent risk factors for CAC in maintenance dialysis patients.5)The results of ROC curve analysis showed that the combined index of Ty G index and age had the greatest predictive value for the occurrence of CAC in dialysis patients,with an area under the curve of 0.807(95% CI = 0.756 to 0.857,P < 0.001),which was higher than the area under the curve when Ty G index(0.689,95% CI = 0.628 to 0.750,P < 0.001)and age(0.762,95% CI = 0.706 to 0.818,P < 0.001)were analyzed separately.Conclusion: High incidence of coronary artery calcification in maintenance hemodialysis and peritoneal dialysis patients.Triglyceride glucose product index,age,dialysis age,and C-reactive protein were independent risk factors for coronary artery calcification.Serum triglyceride glucose product index has a good predictive value for the occurrence of coronary artery calcification in maintenance dialysis patients;and its combined index with age has a predictive value for coronary artery calcification.In clinical practice,it is necessary to regularly detect vascular calcification in dialysis patients,strengthen the management of risk factors,and improve the prognosis of patients.
