Effect Of TERT Promoter Mutation On Prognosis Of HCC Patients After Radical Resection Surgery

Abstract/Summary:
Objective:Hepatocellular carcinoma(HCC)is one of the common malignant tumors in the world,and surgery is the first choice for its treatment.However,because of the high recurrence rate after surgery,the postoperative prognosis of HCC patients is often unsatisfactory.Telomerase reverse transcriptase(TERT)gene is a tumor-related gene,which can enhance the proliferation and differentiation potential of cells by activating telomerase to induce tumors.Mutation of the TERT gene promoter is also considered to be an upstream event in hepatocarcinogenesis.The level of serum TERT mRNA is also considered to be associated with postoperative recurrence of liver cancer.Based on the characteristics of TERT gene,this research hopes to understand its possible application and prospect as a biomarker in clinical treatment by studying the role of TERT gene promoter mutation in the development and prognosis of liver cancer.Methods:This research is a retrospective study.The research subjects were 212patients with HCC who underwent radical resection in Sichuan Cancer Hospital from January 2009 to January 2016,and their postoperative tumor specimens were obtained after consent.TERT gene promoter mutation,TP53 gene mutation and CTNNB1 gene mutation in tumor specimens were detected by Sanger sequencing.The expression of TERT mRNA was detected by SYBR method.Routine follow-up was performed on the patients after operation,and their overall survival(OS)and disease-free survival(DFS)were recorded.Results and Conclusion:According to the detection,the mutation rates of the TERT gene promoter,TP53 gene and CTNNB1 gene in the enrolled patients were17.9%(38/212),40.1%(85/212)and 13.7%(29/212),respectively.TERT gene promoter mutation was associated with Child-Pugh grade and albumin level(P<0.05).The expression of TERT mRNA was correlated with HBV infection,Child-Pugh grade,AST value and ALT value(P<0.05).Cox proportional hazards regression results showed that non-anatomical liver resection,tumor diameter>5 cm and high TERT mRNA expression were associated with poor OS(P<0.05);platelets≤100×10~9/L,tumor diameter>5 cm,TERT gene High mRNA expression was associated with poorer DFS(P<0.05).None of the other metrics collected showed statistical significance.The above results indicate that TERT gene promoter mutation exists in patients with HCC,but it is only related to the liver function of patients,and cannot be an indicator for predicting the prognosis of patients after HCC surgery,nor does it affect the OS and DFS of patients.The expression level of TERT mRNA is an indicator that affects the prognosis of patients with HCC after surgery,and it is related to the OS and DFS of patients.In conclusion,TERT mRNA may be a marker for the prognosis of HCC.
