| Objective:Glaucoma is a group of clinical signs or eye diseases that threaten and damage the optic nerve and its visual pathway and is characterized by progressive optic nerve lesions.It is currently the leading cause of irreversible vision loss.In my country,primary acute angle-closure glaucoma(PAACG)is the most common type of glaucoma patients.The progressive apoptosis of retinal ganglion cells(RGCs)is the pathological basis of optic nerve damage in glaucoma.Therefore,reducing or saving the damage of RGCs in glaucoma patients is one of the main research directions for the treatment of glaucoma.However,the mechanism of RGCs injury is very complex,which makes it very difficult to protect the optic nerve in glaucoma patients.In recent years,studies have shown that a variety of nerve growth factors are related to the protection of glaucoma RGCs.In this study,by detecting the nerve growth factors in the aqueous humor of PAACG patients and age-related cataract patients,including: insulin-like growth factor 1(IGF-1),brain-derived Differential expression of BDNF,pigment epithelium-derived factor(PEDF)to explore the possible role of nerve growth factor in the protection of optic nerve in glaucoma;The nerve growth factors in aqueous humor(AH)of patients with different intraocular pressure and visual field damage were compared and analyzed,to explore the possible influence of the differential expression of nerve growth factor in the AH of PAACG patients with different intraocular pressure and visual field damage on the occurrence and development of glaucoma.Methods:A total of 54 patients with PAACG who underwent surgery in our hospital from April 2019 to October 2021 were selected as the experimental group,and 29 patients with age-related cataracts were selected as the control group.Grouping of the study on the differential expression of neurogenesis factors in AH between PAACG patients and age-related cataract patients: Experimental group: 54 patients with PAACG were randomly divided into groups A and B(age > 50): group A: 26 cases,group B: 28 cases Cases;control group: 29 cases of senile cataract patients were randomly divided into two groups a and b(age >50),10 cases in group a and 19 cases in group b.Grouping of study on the correlation between nerve growth factor expression and visual field damage in patients with AH of PAACG : According to USP-GVFSS,glaucoma patients in group A and group B were grouped according to visual field index(VFI).Group A : VFI > 78 % : 11 cases,VFI ≤ 78 % : 15 cases,group B :VFI > 78 % : 11 cases,VFI ≤ 78 % : 17 cases.grouping of the correlation between nerve growth factor expression level and intraocular pressure in AH of PAACG patients;group A : IOP ≤ 21 mm Hg : 9 cases,21 < IOP < 50 mm Hg : 5 cases,IOP≥ 50 mm Hg : 12 cases;group B : IOP ≤ 21 mm Hg : 6 cases,21 < IOP < 50 mm Hg :12 cases,IOP ≥ 50 mm Hg : 10 cases.AH was collected in all patients about50-100μl before surgery.The concentrations of IGF-1 in AH of PAACG patients A and age-related cataract patients a were detected by enzyme-linked immunosorbent assay(ELISA).The concentrations of BDNF and PEDF in AH of PAACG group B and age-related cataract group B were detected by Luminex multifactor test.SPSS25.0 statistical software was used to analyze the measured data and compare the expression levels of nerve growth factor in AH between the experimental group and the control group.Result:1.The concentration of IGF-1 in AH of PAACG group was higher than that in age-related cataract group,and the difference was statistically significant(p < 0.001).The concentration of BDNF and PEDF in AH of PAACG patients was higher than that of age-related cataract patients,and the difference was statistically significant(p< 0.05).2.The concentration of IGF-1 in AH of PAACG patients decreased with the aggravation of visual field damage,but the difference was not statistically significant(p > 0.05).The concentration of BDNF in AH of PAACG patients decreased with the aggravation of visual field damage,and the difference was statistically significant(P< 0.05).The concentration of PEDF increased slightly with the aggravation of visual field damage,but the difference was not statistically significant(P > 0.05).3.There was no significant difference in the concentrations of IGF-1,BDNF and PEDF in AH of PAACG patients under different intraocular pressures(p > 0.05).Conclusion:1.There were differences in the expression of IGF-1,BDNF and PEDF in AH between PAACG patients and age-related cataract patients.The expression of IGF-1,BDNF and PEDF in AH of PAACG patients was higher than that of age-related cataract patients2.The expression of BDNF in AH of PAACG patients was correlated with the degree of visual field damage.With the aggravation of visual field damage,the expression level of BDNF in AH of PAACG patients decreased.3.The expression levels of IGF-1 and PEDF in AH of PAACG patients were not correlated with visual field damage and intraocular pressure. |
