Objective:To explore the risk factors of recent complications after laparoscopic radical gastrectomy for gastric cancer, and to provide an effective basis for the prevention, diagnosis and treatment of postoperative complications after laparoscopic radical gastrectomy for gastric cancer. Methods:A retrospective analysis of 192 patients with laparoscopic radical gastrectomy for gastric cancer who were admitted to the Department of Gastrointestinal Colorectal Anal Surgery of China-Japan Unition Hospital of Jilin University from August 2019 to August 2021 and met the inclusion criteria of this study were retrospectively analyzed;the general data of the patients were collected: Name, gender, age, height, weight, history of smoking and drinking, history of previous abdominal surgery, whether there is any underlying disease(hypertension, diabetes, coronary heart disease);preoperative related indicators: hemoglobin, serum albumin, lymphocyte count;Medium-related indexes: operation method, operation time, gastrointestinal reconstruction method, intraoperative blood loss, intraoperative blood transfusion or not;postoperative pathological indexes:tumor length, tumor differentiation,TNM stage, number of obtained lymph nodes;postoperative hospitalization complications during the period. All patients were divided into two groups according to the occurrence of postoperative complications. Those with postoperative complications were the observation group, and those without postoperative complications were the control group.SPSS25.0 statistical software was used for data analysis, and the qualitative data were taken as the number of cases.and the corresponding percentages;quantitative data are expressed as mean ±standard deviation;non-normally distributed quantitative data are expressed as median(25% quantile,75% quantile).Univariate analysis was used to screen out the risk factors that may affect patients after surgery, and then multivariate Logistic regression analysis could balance a variety of confounding factors(P<0.05 was statistically significant),and objectively analyze perioperative related clinical indicators and postoperative Complications in relation to the final conclusion. Results:Among all the patients included in the study,92 patients had postoperative complications(observation group),accounting for 48% of the total, including 35 patients with abdominal complications,54 patients with thoracic complications and 3 patients with other complications; There were 100 cases(control group)without postoperative complications. The results of univariate analysis indicated that age, previous history of underlying diseases and intraoperative blood loss were associated with postoperative complications after laparoscopic gastric cancer(P<0.05).The effect was statistically significant(OR=3.151,95%CI 1.690-5.874,P=0.000);the patient’s previous underlying diseases had a statistically significant effect on postoperative complications(OR=2.149,95%CI1.150-4.013,P=0.016);there was no significant effect of intraoperative blood loss on postoperative complications(OR=1.002,95%CI 0.999-1.005,P=0.111);age ≥ 65 years old and previous history Disease history is an independent risk factor for short-term postoperative complications。Conclusions:Age,previous underlying diseases, and intraoperative blood loss were statistically correlated with recent complications after laparoscopic gastric cancer surgery, and were the factors affecting postoperative complications;among them, age ≥ 65 years old and previous underlying diseases were the factors affecting laparoscopic gastric cancer.Independent risk factors for postoperative complications.
