PurposeSerum Afamin(tocopherol binding protein)levels in pregnant women diagnosed with GDM and NGT who underwent OGTT at 24-28 weeks of gestation and the first visit after 28 weeks of gestation were measured.The serum levels of famine in pregnant women diagnosed with GDM at 24-28 weeks of gestation,NGT at 28 weeks of gestation and NGT were observed.The correlation between Afamin and IL-1 β(interleukin-1 β)was further confirmed,providing a new direction for clarifying the pathogenesis of GDM.MethodsA total of 72 singleton pregnant women who were admitted to the Affiliated Central Hospital of Shenyang Medical College from November 2020 to January 2022 for routine prenatal check-up were selected.Oral glucose tolerance test(OGTT,Oral Glucose Tolerance test(ORAL Glucose Tolerance test).Gestational Diabetes Mellitus(GDM)was first diagnosed at 24-28 weeks of gestation,54 pregnant women were diagnosed with GDM after 28 weeks of gestation,and 18 pregnant women with NGT.Maternal serum was immediately extracted and retained during the prenatal examination,and the basic information of the patient was recorded.Then,the pregnant women in the GDM group were taught about diabetes and instructed to exercise diet to control their blood glucose levels during pregnancy(this experiment did not include the intervention group with insulin control).The total daily intake of pregnant women should be determined according to their weight before pregnancy and whether and how much weight increases during pregnancy.Individualized diabetes diet was developed and patients were instructed to measure blood glucose.The results of blood glucose monitoring in pregnant women were followed up by telephone from 37 weeks to 42 weeks of pregnancy.In retrospective analysis,according to the gestational blood glucose control objectives of GDM patients: fasting blood glucose ≤5.3mmol/L,2h postprandial blood glucose ≤ 6.7mmol/L,different gestational GDM groups were divided into glucose intervention group and non-intervention group,and the serum of gestational GDM group whose blood glucose was controlled within the normal range after intervention was detected and compared.The Enzyme Linked Immunosorbent Assay(ELISA)test was used to determine the concentration of afamin and il-1 β,analyze the level of afamin expression between groups and the correlation with il-1 β.Note:Group A: GDM diagnosed at 24-28 weeks(without intervention)Group B: GDM(intervention)diagnosed at 24-28 weeks;Group B ’: full term delivery(after intervention)Group C: GDM diagnosed after 28 weeks(without intervention)Group D: GDM was diagnosed after 28 weeks(intervention);Group D ’: term delivery(after interventionGroup E: 24-28 weeks diagnosed NGTGroup F: NGT was diagnosed after 28 weeksResult1.The comparison of the general data(age,BMI,gestational week and number of births)of GDM and NGT groups showed that there was no significant difference among gdm-a group,gdm-b group and ngt-e group(P > 0.05);The comparison of the general data(age,BMI,gestational weeks and times of delivery)of GDM and NGT groups showed that there was no significant difference among gdm-c group,gdm-d group and ngt-f group(P > 0.05).2.The content of Afamin in GDM group was significantly higher than that in NGT group during 24-28 weeks of gestation and after 28 weeks of gestation,with statistical significance(P < 0.05).3.The concentration of serum afamin in pregnant women with GDM diagnosed at24-28 weeks of gestation and after 28 weeks of gestation was significantly higher in gdm-b group than that in gdm-b ’group(P < 0.05);Gdm-d group was significantly higher than gdm-d ’group(P < 0.05).4.Correlation analysis showed that there was a correlation between serum Afamin concentration and IL-1β concentration in GDM pregnant women without intervention group(R =0.363,P =0.028;R =0.340,P =0.032),and the correlation results were statistically significant.There was no correlation between serum Afamin concentration in venous blood of GDM pregnant women and IL-1β concentration in NGT normal group(R =0.160,P =0.135;R =0.163,P =0.156).Conclusion1.However,the serum Afamin in GDM group was significantly higher than that in NGT group,however,the increase in non-intervention serum famin,which is well above the target value of the group,is a disadvantage that causes this production.2.Serum Afamin concentration was correlated with IL-1β concentration in THE GDM group,and the correlation results were statistically significant,but the specific mechanism of the effect needs further discussion.
