| ObjectiveA total of 82 patients who were diagnosed with adenomyosis and had undergone hysterectomy under the appropriate laparoscopic range were selected to study.The data were collected and retrospective analysis by statistical method.Laparoscopic subtotal hysterectomy(LSH)and laparoscopic total hysterectomy(LTH)were compared between the two surgical methods,analysis of its differences in the advantages and disadvantages of multiple aspects of treatment of adenomyosis.And finally concluded the differences of clinical efficacy and other aspects between the LSH and LTH,In order to provide a reasonable basis for patients who are suffering from adenomyosis to choose laparoscopic hysterectomy.MethodA total of 82 patients underwent laparoscopic hysterectomy who were diagnosed with adenomyosis in the Affiliated Hospital of Taishan Medical College from December 2013 to June 2015 were retrospectively analyzed.According to the different surgical methods,there is laparoscopic total hysterectomy(LTH)group,a total of 34 cases,and laparoscopic total hysterectomy(LSH)group,a total of 48 cases.By comparing the time of operation,the amount of bleeding during operation,the time of postoperative anal exhaust,the length of stay,the time of indwelling catheter,the pressure of pelvic pressure,pain score,complication,pain relief,postoperative quality of life,postoperative chronic pain incidence,cervical stump diseaseand so on.Through analysising the above data,evaluate the two surgical methods to explore its clinical value and significance in the treatment of adenomyosis.Result1.After statistical analysis,we found there was no Statistical significance between the two groups in age,body weight and hemoglobin content(P>0.05).2.LSH group operation time was(90.54±8.46minutes),which was Less than LTH group operation time(128.79 ± 15.23minutes),the difference has statistically significant(P<0.05);LSH intraoperative blood loss(55.58±13.48 ml),which was less than that in LTH group(63.44± 8.27ml),the difference has statistically significant(P<0.05).3.Average hospital days was(4.19 ± 0.39days)in LSH group,which was shorter than that in LTH group days(5.44±0.50 days),the difference has statistically significant(P<0.05);placed catheterization time was(1.60 ± 0.49days)in LSH group,which was shorter than that in LTH group(2.21 ± 0.41 days),the difference has statistically significant(P<0.05);Postoperative anal exhaust time was average(1.69 ± 0.62days),LTH group average time was(1.85±0.59days),were not statistically significant(P>0.05).The difference has no Statistically significance between the two groups(P>0.05);The postoperative pain score of LSH group was(5.21±1.15points),the postoperative pain score of the LTH group was(5.06±1.18 points),the difference has no significant between the two groups(P>0.05);The postoperative drainage fluid of LSH group was(125.10±17.05ml),the postoperative drainage fluid of LTH group was(126.47±24.48ml),the difference has no significant between the two groups(P>0.05);In terms of the incidence of postoperative complications,LSH group was lower,it’s about 4.2%,LTH group is higher,it’s about8.8%,although the incidence of complications in the LSH group was lower than that in the LTH group,but after statistical analysis,the difference between the two groups did not show statistical significance(P>0.05).4.The rate of remission of patients’ dysmenorrhea situation in the two groups was significantly higher,there was no statistically significant difference between the two groups(P>0.05),the incidence of chronic pelvic pain in the LSH group was lower than that in the LTH group,it has statistically significant(P<0.05);The rate of postoperative sexual satisfaction of LSH group 6mounths,12 mounths and 24 mounths respectively was 87.5%,85.42%,85.41%,it’s much higher than the rate of postoperative sexual satisfaction of LTH group 6mounths,12 mounths and 24 mounths respectively was 67.64%,64.71%,61.77%,the difference between the two groups was significant,All of the number was P <0.05,it has statistically significant.Conclusion1.Both LSH and LTH have a better effect on adenomyosis,mainly in the obvious relief of menstrual pain.Maily performed in relieving menstrual pain obviously.2.Compared LSH group with LTH group,there was a significant reduction in bleeding volume during surgery,significantly shorter duration of surgery,significantly reduced hospital stay,shortened catheter retention time,low incidence of chronic pain after surgery,high sexual satisfaction.3.There was no postoperative cervical lesions and cyclical vaginal bleeding and other complications after the operation of LTH group. |
